  • In this first episode of a four-part Double Take video miniseries on depression from the New England Journal of Medicine, Dr. Scott-Vernaglia recounts her personal experiences with depression. She and Drs. Sanacora, Crawford, Teel, and Anderson discuss the prevalence of depression and the need to empower the primary care physician to screen for, diagnose, and address suicidality. Available therapies for treatment-resistant depression are briefly introduced and will be explored in greater depth in episode 2 of the miniseries.
For further reading, the following articles, referenced in the video, are available on the Journal’s website: Treatment-Resistant Depression in Older Adults (Steffens, in the February 15, 2024, issue); Ketamine versus ECT for Nonpsychotic Treatment-Resistant Major Depression (Anand et al., in the June 22, 2023, issue); and Electroconvulsive Therapy (Espinoza and Kellner, in the February 17, 2022, issue).
The New England Journal of Medicine is the world’s leading general medical journal. Continuously published for over 200 years, the Journal publishes peer-reviewed research along with interactive clinical content for physicians, educators, and the global medical community at NEJM.org.

I have suffered with depression for a long time; from the age of 25 to 45 I have self medicated with Sertraline and huge amounts of Alcohol; I don't know why I am not dead; my brother died of liver failure at 32.
Sertraline is shit; it gives a brief boost; then it wears off and demolishes your sex drive (which is fundamental to our nature); and then you have to go through 'discontinuation syndrome' aka withdrawl; though of course it's not addictive?
Recently, however, I have come to understand that our society is sick; and depression is not an irrational reaction to that. I intend to earn as much money as possible to help people with severe mental health issues.

As a PCP, I can say that referrals to psychiatrists NEVER work. There are none available and even when there are they don’t accept referrals from PCPs.
