Apparently it comes from rhyming slang, "Dicky dirt" = shirt. So presumably a bow to go with the shirt. I don't think anyone uses the phrase now though.
We also say dicky bird to refer to any small bow tie sized bird. Photographers would put a bird on top of their cameras and say watch the dicky bird as they take a photo. The only other use I can think of is if you have heart problems you would say I have a dicky heart.

Maybe it's used when someone wants to be slight patronising about how someone has dressed up in a smart way. "Look at yooooo! Don't you look smart..."
