    Sir what if patient is not able to do the 3rd and 4th .
    In that case would we continue trying and wait for the movements and strength to increase ??

      Very true and at the same time we would also incorporate exercises utilizing the following Neurorehabilitation secrets:
      1. Utlizing the end range and isometric to eccentric contraction
      2. Using CKC activities to facilitate co-contraction ( stability)
      3. Spindle biasing ( positioning for prolonged period of weaker joints/ muscles in such a manner so as to make their spindles shorten and more sensitive to stretch and resistance)
      4. Perceptual motor control training ( integrating feeling with moving)
      5. Environmental integration
      Etc.

    Sir I have doubt *patient has spasticity on their hand or brachioradialis and patient unable to move his arm so can we apply rhythmic initiation Technique on him because sir spasticity various with velocity so that's why I have doubt..?

    3rd step should be active and then with resistance 4th??
    After several repetitions of active movement resistance can be given to reinforce the movement

      Resistance is the strongest proprioceptive input a therapist can give to make the patient's brain understand the movement and gain control over the muscles required to fire the movement.
      Resistance to a weaker muscle is given in a shortened position and in end of range to start with.
      Active movements form the last stage as the therapist removes his/ her proprioceptive cues and makes the patient perform the exercise on his own.

      Rhythmic initiation is a concept of exercise profession to improve patient's motor control.
      Progression from one stage to other will be made only when the necessary prerequisite has been met.
      It will many a times not possible to complete all phases in one session.
      Focus initially to improve motor sensation and by the 1st phase and as you find patient can relax and has the ability to participate as him to contribute in the exercise which will be second phase.
      Progression to 3rd phase will only be made when patient develops the ability to contract and relax the target muscle groups.

      In summary : Number of repetition is never fixed in NEUROLOGICAL rehabilitation contrary to musculoskeletal rehab in which we can have a fixed number of repetitions.

    Content is solid. Great work. My only complaint is that the movement doesn't follow a PNF pattern. Getting the patient to cross the body is important and shouldn't be overlooked. I didn't give the dislike in case others are wondering.

      The PNF series has been made with the intention of making its concept simple to understand and it's techniques easier to practice.
      Even the PNF books mentions that it is not necessary to know PNF patterns to practice PNF.
      If a Physiotherapist can analyze a functional movement and it's kinematics, then also PNF can be used effectively. 🙏

    Hii sir i really liked this video..and all the other pnf videos hv helped me alot....but i hv a confusion in this one... its in 3rd n 4th step...you said 3rd is active resisted and 4th is fully active....whereas according to my knowledge.....1st is passive
    2nd is active assisted
    3rd is active
    4th is active resisted....
    Pll clarify this if i am wrng

      Here Active resisted is coming before Active to give more proprioceptive input to the patient's target muscle or joint.
      Resistance will help in improving the spindle activity and making the patient's brain more aware about the direction of motion.
      Once the patient learns to activate the desired muscles in the right direction, then we progress to active phase where we stop giving therapist induced proprioceptive input.
