    I truly appreciate how you break things down on this video. I never thought of writing down the kind of repairs down with measurements. It really is helpful. I just hope I dont take so much time during exam over thinking it. These cases comes up a lot in the COC exam which I am taking soon again.

Without your videos; I couldn’t have been able to understand the fundamental knowledge of what ICD 10; Cpt or PCS are.       Hi Phung. Thank you so much for your kind words. You make me feel appreciated and want to do more. What I am doing now is also to prepare you for the CPC and other coding exams. My plan is to cover all important chapters of ICD-10-CM and some basic CPT coding guidelines first then I will switch gear and focus more on CPC exam. If you follow my plan, then you should be ready to take the CPC exam this year. Slowly but surely, you will get there. Have a good week !

    My plan is to take the CPC exam in May. I work in managed care in utilization review and want to add coding to my skill set and obtain a PRN position. I am following your videos and going back and re-reviewing them as I think they will be very helpful to remember the tips for the exam.

    Mr. Hoang I agree with so many of the comments on this video and the others you've taken so much time to prepare for us. I have struggled with many of the coding guidelines, not able to understand them or remember them. This section is one of them. It was very confusing to decide and separate the wounds, the repair complexity and to add or not to add together the lengths. I almost gave up the thought of being able to code after trying for so long.
    I have a question and that is (for all coding) how do you determine the order of the codes?

      When a patient has multiple diagnoses, assuming that we treat them all, then the you list the most important or serious condition FRIST. For example, an open fracture of the right femur and a closed fracture of the left femur were treated on the same encounter, based on the severity of the illness, you have to list the open fracture FIRST followed by the closed fracture. In some cases, if you are coding neoplasm, HIV or Pregnancy case etc., then you have to follow its chapter coding guidelines. If you watch all my videos then you should know some of them by now. I hope it helps.

    Mr. Hoang, Thank you for this informative video. Very well done! Question on the last example, for intermediate closure would go first then the other two codes. After the IC would you code in numeric order (ie. 12034, 12004, 12011 or would it be 12034, 12011, 12004) or does it not matter?

      Hi Tfryeba. Based on the severity illness rule, you list the intermediate repair before the simple repair or a malignant lesion before a benign lesion or an open fracture before a closed fracture and forget about the numeric order. I hope it helps.

      @@codingwithhoang, but in this particular case you provided, 12011 and 12004 are both simple repair codes. Would 12004 be considered the more work intensive code because the sum of the lengths repaired is greater (8.6 cm.) than the sum of the lengths repaired in code 12011 (2 cm.)? If I am correct, the codes would be sequenced as follows: 12034 (intermediate), 12004 (simple, 8.6 cm.), 12011(simple, 2 cm.)

    Dear Doc, For the complex repair in the 2020 CPT book, begins with 13151 not 13150. Just thought I should mention this.

      I don't have a CPT book at work, we use software to assign codes so I still use my old CPT book at home. Anyone who already passed the CPC exam in 2020 kindly donates his or her CPT book to me, that would be great. Otherwise, I have to search for a used book to buy in order to prepare for these video lessons going forward. Thank you.

    Hi Sir Hoang, 19:28, I would like to confirm, reporting of the codes in real world coding, would be 12034, 12004-59, 12011-59?
    From the guideline of repair #2 second paragraph, when more than one classification of wounds is repaired, list the more complicated as primary and the less complicated as secondary using modifier-59.

      Hi Deepa. In code 12031 the extremities refer to upper arm (shoulder to elbow), lower arm (elbow to wrist) and lower extremities (hip to ankle). If the repair is performed on the hands and feet then you have to use 12041. I believe AMA separates the extremities with hands and feet simply because it is much harder to repair the wound on the hand and feet due to its density of nerves, ligaments and tendons compared to skin and subcutaneous tissue of the upper arm or thigh. I hope it helps.

    Hello doc I have a question what is considered extremity and what is considered feet. The reason being with this scenario Intermediate repair 12031 Repair of of scalp, axillae, trunk and or extremity (excluding hands and feet) and intermediate repair 12041 repair intermediate neck hand, feet, and or external genitalia. So I chose 12046 because I considered leg to be feet/foot. Please provide clarity. Thank you Doc.

      CPT has two separate codes for the intermediate repair of extremities and intermediate repair of hands and feet. In order to code these procedures correctly, you got to know the difference between extremities which include both upper extremity (upper arm and forearm) and lower extremity (thigh and lower leg below the knee). Wrist, hand, ankle, and feet will share the same code starting 12041 for intermediate repair. I hope it helps.

    when coding wounds and seuquencing you said most intense 1st....you coded 12034, what would go next 12004 or 12011. 12011 is smaller so would that code not go last? You had it second in video

    MR hoang 12031 cpt code states repair intermediate wound of scalp , axlliae , trunk and/or other extremities ( excluding hands and feet ) 2.5cm or less , meaning hands and feet are excluded , i think the code should be 12041 because it states repair intermediate wounds of neck, hand ,feet and/or external genitalia 2.5 cm or less..... so why is the intermediate repair answer in the first question 12036 ...... i am little bit confused with the ( excluding hands and feet ) part ?

      12041 can be assigned for intermediate repair of the hands and feet but not for the LOWER LEG and that is why CPT has a separate code 12031 for the extremities (upper arm, forearm and lower leg). In this case we have to use 12036 because the wound size is 25 cm. I hope it helps.

    Mr. Hoang I was a little confused with the 1st case. It read that the wound was to the muscle but you coded it under Intermediate instead of complex. Can you explain that to me? Thanks

      Hi Angie, the physician has to document it as a complex closure in order for us to use the complex repair. In this case, it's documented as layered closure. It does not matter how deep the wound is if it is closure by two layers then you have to use the intermediate repair. I hope it helps.

      13150 is for a complex repair, eyelids, nose, ears and/or lips; 1.0cm or less. This code has been deleted and now it starts with 13151 repair, complex, eyelids, nose, ears and/or lip; 1.1cm to 2.5cm. That said, for any complex repair of eyelids, nose, ears or lip less than 1.1cm, you have to code it either simple or intermediate repair 12011 or 12051 depending on how many layered sutures. I hope it helps.

      Yes, I would not use the simple repair code(s) for adhesive strip and derma bond. I would add the work to the E/M level 3: 99283 instead of 99282 for the ED visit.
