Getting an S rank overall doesn't seem too bad, but getting an S+++ combo seems impossible. I've been going through the firefight levels, trying to do all the challenges, and the challenge in "Cipher" for getting an S+++ combo seems like I either need a bunch of enemies to line up for me (which they don't), or I genuinely need to have superhuman reaction times, especially since the slow-mo doesn't seem to slow down the combo meter decay. Kind of defeats the purpose of having the slow-mo imo. Even here in a fairly open level, you were having a hard time getting to S+. I'll get up to an A, maybe an S, before either I'm suddenly out of enemies in my immediate area or I take a second to think and suddenly I'm back at C rank, and I have to get three whole ranks higher than that? Obligatory "skill issue" yeah yeah, but the challenge genuinely feels disproportionately difficult compared to the other challenges that are like "dive through X windows" or "kill X enemies while upside down". I've tried messing around with the mutators, but none of them really help with combos much, other than the triple spawns one. Though that only really gives me more "attempts" per run since the spawns are staggered. I haven't seen anyone even mention this anywhere though, so am I just missing something? Is there some kind of trick to it? I'm trying to do different things, juggle the enemies, dive through windows, throw, steal weapons, etc. but it really seems like I need the stars to align for it to happen.
Yeah I'm stuck on this same challenge, I've gotten up to S++ but only rarely. I'm gonna try adding back the enemy "outline", which I removed, to hopefully help a bit more with spotting the enemies. Blasting a hole in the ground to get to the lower floor instantly helps btw. Any luck since you posted this comment?

Was struggling to S rank some levels for so long lol, didn't know how much "fresh" adds to the score, I was just trying to brute force it lol
