Good question. However, note that "ESRD undergoing dialysis" is specifically listed under ASA 3, while "ESRD not undergoing dialysis" is under ASA 4. ESRD is excluded from the end-stage organ dysfunction listed in ASA 5. Remember that ASA 5 is reserved for a patient that is bound to pass away without immediate medical intervention, and a patient undergoing dialysis routinely would not fit under that category.
A patient on levothyroxine 50mcg/day, other than that everything is under control or normal is it ASA 2 to ASA 3. Not sure what falls under symptomatic thyroid dysfunction.
That would be an example of ASA 2 if the patient is showing no symptoms of either hypothyroid or hyperthyroid. We'll cover those conditions in more detail hopefully in a future video!

Great Vid Ryan! Quick question: how would you classify an otherwise healthy ASA I patient but has a non-threatening allergy?
An active allergy would put the patient in ASA II. As I said in the video, a true ASA I with no cautions for dental treatment is very rare!
