S5.2: SPEAK! Responds to a paper on Human Error
State of science: evolving perspectives on 'human error'
This week Crystal and Elisa read and respond to a research paper on the evolution of Human Error in the ergonomics and human factors space!
We talk a bit about the authors, how to tell if a paper is worthy of your time, and have a bit of a whinge about how much time it can take to get through some of this stuff! Plus our aha moments and takeaways from the work.

S4.3: Simon Cassin - Hey Safety, how ethical are we, really? 🤨
มุมมอง 415 หลายเดือนก่อน
Just because I ought to…does that mean I can? Just because I can…does that mean I ought to? How does ‘can’ and ‘ought’ relate to human factors, and to safety? Are we as ethical as we think we are? What’s your argument for that? Simon Cassin joins us to pose some questions that maybe only a philosophical approach can answer. Elisa here: Part way through this episode, the conversation very briefl...

    Great discussion. Good point on the idea that risk assessments seem to be stuck in the past Crystal. None of the teams who need to know about the risks, are going to sit down and understand a (generic, or mostly irrelevent) 20 pages risk assessment before they start to work. Risk assessments don't seem to have advanced as much as other areas of business.

    I think morality and ethics are quite seperate but often confused ideas. Especially the distinction between personal morals and professional ethics. The ethics of the health and safety industry are questionable to non existent. If there was a professional ethic underpinning the health and safety industry could we 'fudge' injury rate data?

    I have managed so many people who probably experienced these challenges at work and I'm finally learn that this is not something I have ever considered. Thanks for sharing I learned a lot. We need to do more to educate managers around these topics and to take away the stigma which exists.

    Great podcast! I talk to a large number of young people considering H&S as a career. I'd love to see those stats which show the surge of retirees, as this will help highlight the opportunity in these conversations. Is there a public link to the article/report/stats?

    I’m so aligned with your discussion.. I’ve been a Health and Safety practitioner in the construction industry for close to 30 years. Is Zero Harm a policy or a vision? Here’s a couple of analogies: not my words, just examples I’ve heard and like….. I once saw a squirrel fall from a tree. What have squirrels evolved to do? Jump between trees to survive…. And every now and then, they miss!! And squirrels only really have jumping between trees and survival to think about…. When reviewing incidents at the end of the financial year. Commercial Manager states, “I see that last year 10 people fell over and were injured when walking backwards… we should ban people from walking backwards”. The Safety Manager states… “I notice that 30 people were injured falling when walking forwards… perhaps we should ban walking forward too!!”. To answer on of your earlier discussion points the HSE (UK) RIDDOR guidance defines what is a “work related accident”, a twinge, for example, is not work related unless the work activity or condition of the site or premises contributed to the accident… and I’m not making reasons not to do anything about such incidents, just pointing out that we may over report such incidents in certain highly regulated industries, where in other areas 60% of incidents go unreported. www.hse.gov.uk/riddor/key-definitions.htm

    All I heard here is true.... I'm one of those front line workers, have been all my life, largely because I enjoy it and am good at it. I've seen and been personally involved in all the wrongs that you guys talk about. Guys that had pinch/ crush finger injuries that turned up a couple of hours later with a bandage, driving a broom with one hand so there is no LTI. Total disengagement regarding near miss/low level injuries, so the zero harm doesn't get stirred up (from workers not reporting) Actively being down graded to " it's just a scratch," when realistically way worse. A total sham, and the guys on the ground all know it. I suspect most people at more senior levels do too, they all just continue to play the game, so that ultimately, in court, they can present a whole bunch of paper,stats and other lies to not end up being found guilty. A great chat, thank you!

    Is a fatality a failure if you have been prosecuted and found not guilty - you did everything reasonably practicable?

    • @speakunedited
      @speakunedited ปีที่แล้ว

      Hi Greg, I don’t think there is any scenario where losing a life is not a failure of some sort. It’s a very good question!

    • @gregsmith8504
      @gregsmith8504 ปีที่แล้ว

      @@speakunedited I think it is an interesting challenge, because there is no obligation to prevent all accidents, and fatalities have been successfully defended in the past. Also because, there will always been fatalities with some connection to work over which the organisation has no control or influence.

    really enjoyed this, a lot of the points raised resonated with my thoughts. I like to flip the zero 'harm' to what does good look like?, how do you get people to bring the best of themselves to the workplace?
