Toilets were my successful run MVP. There was a period of time where I seriously thought I'd never get a successful playthrough xD Yes to basically everything! More anomalies per loop, more that you have to get right, and entities are way more frequent. We didn't even unlock "hard" until we beat normal and there is an even higher difficulty if we beat that. Also, I never got got by the entities (brag) or I would have included it. So, I don't actually know. I imagine there would be a jump scare and the Anom counter would reset to 0 and start you at the beginning of the loop? I'm not sure, but there is also an achievement for burning the dolls of the entities? The one I hated was Carrotera and I certainly wouldn't mind the catharsis that would bring.

The red car did save me! I still don't know why, but I appreciate it nonetheless. At the end, the red car is also following us after we escape and it feels nefarious. Maybe the red car saved us just so we could help it escape?
