ooo this is so good! YES! Excess aldosterone could absolutely cause an alkalosis by promoting hydrogen loss in the collecting duct - so could absolutely be a feature in a patients with Conn's.
I have a C-ANCA diagnosis and am negative for MPO and RP3. In the past I have been infected with the Epstein Barr virus. I do have mild abnormal liver values. In 2 weeks I have an appointment with a liver specialist.

Ma'am, how to distinguish between Classical pathway and Lectin pathway, based on C3 and C4? Because both pathways are almost similar. Classical uses C1 as the first complement whereas Lectin pathway uses MBL. So, are C1 levels detected?

Yes!!! This is correct but not enough to light up a dipstick. And we have a normal protein in our urine called tam horsfall protein or uromodulin which makes up this normal range of protein. And like you say tiny wee drop of microalbuminuria on formal lab testing makes up normal range :) totally spot on

Hi Kristeen, what do you tend to do when a pt presents with fluid overload and there’s no past renal baseline for comparison, and their renal function is deteriorating with diuresis but they still appear clinically overloaded? Is there a percentage of say 30% increase in creatinine that would be acceptable for a ‘new renal baseline’ when we accept the increase in renal biomarkers? Thanks so much for your videos!
This is a great question. It depends on the unique case but aiming for 0.5-1kg negative balance usually causes only a mild change in gfr due to diuresis, whereas if the gfr jump is large it might be that the diuresis was too much in a 24 hour period. So that’s the first thing. The other thing is that usually the gfr if it drops a little during diuresis, it stabilises out, and it shouldn’t keep going off, that would definitely make me cut back. And of course all of the Aki work up still applies to these patients, wouldn’t just put it down to diuresis if gfr changes significantly. Percentage values are hard to give here, I think patterns and context and the whole picture need to be considered with diuretic effect in the differential alongside other things to be safe.
