PA School Pharmacotherapeutics: PDE5 inhibitors: Treating Erectile Dysfunction
มุมมอง 666ปีที่แล้ว
Treating Erectile Dysfunction heavily relies on the use of a class of medications called PDE5, or phosphodiesterase-5 inhibitors. These medications allow the smooth muscle inside of vessel walls to relax, allowing the dilation necessary for erection to occur. Importantly, this vasodilation is also prolonged by the inhibition of an enzyme that breaks down and ends the dilation. We review the mec...

    You're right, I just realized that Mario updated his favorite way to tackle ED and it's crazy! Although what he previously talked about was pretty decent, it was difficult to follow, I just go'ogled the latest in Mario Volpstein's Erections at Will, it's so much simpler and potent now!

    Hi Dr, in the case of Pectus excavatum, which test will demonstrate that the lungs can not expand well? CT? It should be evident through images?

    • @KimberlyBSPhD
      @KimberlyBSPhD ปีที่แล้ว

      Yes, what you want is access to the distance between the spine and the sternum. Combining respiratory function tests with imaging of the chest wall gives you a good picture.

      @@KimberlyBSPhD Thanks! Could you pls explain a little more why that distance is important in order to check how restricted are the lungs to expand? How that distance is to be interpret? Cheers

    Can I ask something? How does inhibitory neurons work? Example, How do dopaminergic neurons having d2 inhibitory receptors transmit signals? Because they will inhibit signal and the next neurons will not be able to recieve signal. How do thet transmit signals to the next inhibitory neurons?

    • @KimberlyBSPhD
      @KimberlyBSPhD ปีที่แล้ว

      Good question! It can get complicated. Remember that in order to send a signal, a neuron has to be ‘excited’ - it needs to have enough positively charged ions enter it to excite it. An inhibitory receptor halts release of a neurotransmitter from a neuron - so the lack of neurotransmitter means there’s nothing released, nothing to bind to the next cells. That LACK of neurotransmitter then is the signal. A neuron can’t get a message to fire if there’s nothing there to turn it on.

    Stop selling this shit! Should only be used a few times for 2 to 4 weeks at the most otherwise dependency can develope and then you are in for possibly years of tapering so as not to freak out and off yourself! Trust me I'm there now! Worst shit I've ever been on. I've lost my job, my license, car and nearly my life! Long term use is linked to Alzheimer's and Dementia as well as higher rate of Depression and Anxiety, Hypertension and Diabetes. This garbage has been said to be the worst crap to get off of by heroin, opiate and alcohol users on many forums and groups! Don't take a chance!

    • @KimberlyBSPhD

      I’m not a clinician, so I can’t make any guesses as to it being a dose issue at the muscle level or the global fall risk involved.

    Why hasn't this information been used to treat PMDD yet?

    • @KimberlyBSPhD
      @KimberlyBSPhD 2 ปีที่แล้ว

      That’s a fantastic question. Unfortunately, there are so many inconsistencies between patients with regards to symptoms experienced and physiologic markers, the consensus seems to be that a clear cause and presentation is yet to be defined. The majority of what I’ve read (which isn’t a ton) is attempting to determine a cause to then identity a treatment. Here’s an interesting study following women through their cycle and measuring mood and stress that should be open access! pubmed.ncbi.nlm.nih.gov/31357135/
