Grief and the Holidays when you’re Terminal
Being informed that your time is short is devistating. Managing the stress of the holidays is challeging and the added worry knowing you are Terminal increases this significantly. Patients facing the Holiday Season after being told that this will more likely than not be the last one, evokes strong emotions. Sometimes, people find a way to find joy, peace, and hope as they express their love to others. Patients that manage to navigate the powerful emotions that are common facing this type of difficult situation report an unexpected freedom. There can be a reduction in typical holiday stressors as patients are given unexpected privileges and liberties by loved ones. There are terrible days and better ones, as anticipated during this stressful time of the year. I hope if this situation is facing you or your loved one, that you find some helpful information here to lighen you burden.

    I am so happy to see Sarah after the last 40 plus years when we were in residency training. (I was a former pathologist who decided to go into family practice at age 50.) Now in my 90's I continue to follow theses issues and enjoy Dr. Bohn's (Sara as I knew her} comments. As we used to say "You go girl".     Very helpful video! I only wish there were "Death and Dying" consultants that one could sit down with. So many legal things need to be done, like wills, trusts, etc. and there are so many questions to ask and nobody to help pull it all together.

      You are so right! Our time can be consumed with the regular daily activies, needs for medical visits, imaging and other tests, taking medications and squeezing in a bit of rest that the 'Mortal' concerns can get overlooked. Being ill is ver difficult!

    My special-needs 23-year old son was diagnosed recently with benign tumors in his liver and the MRI stated these were Focal Nodule Hyperplasia. My son is non-,verbal and we have no communication with him. I would love to know what to expect for him as I'm very concerned.

      Benign Nodular Liver nodules is not as worrisome of a diagnosis as many other liver issues. If the diagnosis is correct, these lesions are often incidental findings with more concern than actual risk associated. Typically these are uncovered in the process of looking for another medical concern. They can but are not likely to change over time. Routine follow up on a regular schedule basis is suggested. It is important to consider the What If’s and how a change might be addressed. This is a very special and unique circumstance that you have outlined. Please discuss this with the Primary Care Team as well as the GI specialist involved to outline the appropriate plan and consider decisions should something adverse occur. I will keep your son and your family in my thoughts. I hope this concern remains only a concern. I admire your work caring for him and thinking of his wishes.

      Think of it as the medications are trying to unravel a process that is well underway. Not a light switch that can be simply turned off. The process of Lupus has caused several of your body systems to react differently than they have in the past and those changes have to be undone. The medications you have been given are working to undo those changes and that just takes time. Hang in there. I hope the best for you and that you have a dedicated Team to help you. Lupus is a life long journey that needs regular attention. Best wishes.

    Thank you for this video. There is a very important and common illness, one of the most horrific connective tissue diseases there is, called Chronic Lyme Disease. So horrific are the infections, co-infections, worms, and parasites, that medical students are taught the disease does not exist, Mayo doctors write in their clinical notes why diagnose this patient when their disease may be difficult to treat before abusing us, the blood test for Lyme was proven by investigation to be designed to be seronegative regardless of the patient’s condition, and no conventional doctor or clinic will help these patients unless we present with Palsy and then they must. The public doesn’t know about chronic Lyme disease and how awful it is. Getting a diagnosis can take ten years, and we are just dead people walking until we die. My Lyme disease caused severe, runaway concomitant fibromyalgia which doctors don’t even believe in. Please help educate the public and help save lives by including chronic Lyme disease in your discussions of connective tissue disease. Thank you so much.

      Chronic Lyme is definitely a real disease process. Although this is not commonly listed as the cause of death on a death certificate, it certainly causes debility, disability and has significant symptoms of suffering associated with this process.

    I have had a diagnosis of Raynauds Dx, Fibromyalgia, Mixed Connective Tissue Disease with a Kidney transplant due to IgG, and IgM chronic kidney disease. Ive had a Stroke with TpA given, and I returned to work as the only Case Manager in a busy 40 bed Neuro/Cardiac/Medical ICU. I continued to work FT for 3 years after my transplant. Then I developed GCA for 2 years and went onto LTD at 60 years of age. I moved back to my home: British Columbia, Canada and I cannot find a knowledgeable Rheumatologist. My last one fired me and the only one who would take me on is 26 years old. Oh I Also have a DVT with 2 small PE's, they are permanent as they were not caught in time for TpA so I'm on permanent blood thinners.

      I am sorry you had this experience, unfortunately it is all too common. These rushed discussions irk me. True, there is plenty of paperwork that needs to be completed and a limited time to do it, but there has to be a better way. Discussions about code status is a very important conversations and deserve time. Kudos to you for learning more about the different life sustaining treatments that are available to help you advocate for your loved one. You have given me an idea to talk about this.

      I am so sorry. That is really difficult news to learn. I hope that you are able to have many good years even though you have liver damage. Some areas have Palliative teams developing and when you are eligible, consider Hospice. You can always ‘revoke’ hospice if you don’t like the service and if you become a candidate for treatment and your prognosis improves, they will discharge you. Neither of revocation nor being discharged costs you and you can sign up again without penalty. Best wishes.

      I am truly sad if the Chaplain did not convey his compassion, commitment and his ability to convey his ability to grasp and address the spiritual concerns and fears of a dying patient. It might have been my questions. Each Discipline does work very hard to do all we can to learn from each person and their loved ones how we can best support them on their own journey. It truly is personal, sensitive, intimate, frightening and utterly devastating for each individual in their own way. Thank you for your comment. Constructive criticism helps us learn.

    ❤I had cataracts removed from both eyes two weeks apart. I paid to have the special lenses so I don't have to wear glasses. Love waking up in the morning and being able to see right away. Glad you had a good experience too.

    Praying for a very successful surgery and comfortable and easy recovery! Would be interested to hear how it goes. I am 45 and have the beginnings of cataracts in both eyes due to steroid eye drops used to control chronic inflammatory flare ups . Question for when you’re recovered - Do you have any experience with Ehlers Danlos patients? When I saw a geneticist a few years ago, he told me EDS won’t be the cause of my “un-a living”, but my physical condition will continue to decline. He even drew a picture of a stair case with big arrows down. Fun image to have seared into my memory forever 😂

      I’ve had a great experience, so far, with the cataract surgery, one to go. Scheduling around everyone’s availability has been challenging. Thank you so much for the well wishes! Without full knowledge of the medical condition of the specific person and review of all medications, it is not possible to provide sufficient safety information and relevant medical information. Zopiclone (Zimovane) is metabolized by the liver through CUP34A and CYP2E1 pathways and any limitation to these by any cause will impair the hepatic drug processing. Elimination half-life for this medication is listed as range of 5-10 hrs based on multiple individual factors of healthy persons. Any person with liver impairment will not be able to process and eliminate this medication normally and drug accumulation is highly likely and can result in toxic and possibly lethal side effects. Long term studies regarding hepatic injury have not been conducted. The risk for liver damage caused by appropriate use with correct dose and monitoring has not been reported to be significant number.
