You got the Kyle Walker part wrong. It's not just about being in position, concentration matters too just like you said. Kyle mentioned that he was instructed by the referee to calm his players down, which he was doing and hence not paying attention to the ball, then came the attack that led to the goal. The referee should at least have given both captains 3-5 seconds to do what he asked of them instead of resuming play immediately.
