How to measure CVP using manual manometer (Bangla)
CVP বা central venous pressure জিনিসটা shock এর পেশেন্টের cause এবং treatment এর ক্ষেত্রে guiding tool হিসেবে কাজ করে। CVP তিনটা পদ্ধতিতে মাপা যায়। প্রথমটা crude method যেখানে USG দিয়ে Inferior venacava র diameter দেখে idea করে নেয়া হয় । দ্বিতীয়ত হচ্ছে electronic cvp transducer দিয়ে । আর তৃতীয়ত manual CVP manometer দিয়ে।
CVP র নরমাল রেন্জ 8-12 mm hg I নিচের কিছু করানে CVP বেড়ে যায় -
Cardiac tamponade
Decreased cardiac output
Forced exhalation
Heart failure
Hypervolemia
Mechanical ventilation and the application of positive end-expiratory pressure (PEEP)
Pleural effusion
Pulmonary embolism
Pulmonary hypertension
Tension pneumothorax
এবং CVP কমার কারন হচ্ছে -
Deep inhalation
Distributive shock
Hypovolemia

CVP র নরমাল রেন্জ 8-12 mm hg I নিচের কিছু করানে CVP বেড়ে যায় -

ইনসুলিন এবং ডায়েবেটিস সহজ দুটো শব্দ এবং একই সাথে ইনসুলিন ম্যানেজমেন্ট টা আমাদের কাছে খুব একটা সহজবোধ্য নয়। আমি নিজেও বহু গাইডলাইন এবং জার্নাল ঘেটে পুরো বিষয়টার কনসেপ্ট কাজ চালানোর মত পর্যায়ে এনেছি। সেটার উপর ভিত্তি করে BES এর গাইডলাইন ফলো করে একটা ভিডিও সিরিজ করছি। আজ রইল চতুর্থ পর্ব। এখানে রইল Basal & basal plus insulin regimen । বোধগম্য হবার জন্য আগের পর্বগুলো দেখতে হবে।

Pacemaker implanted একটা unconscious patient যার সাথে কোন attendant নেই past history দেয়ার মত , তার ক্ষেত্রে ECG দেখে আমরা কি পাবো। মোটামুটি যাদের basic ecg knowledge আছে তারা সকলেই আশা করি উপকৃত হবেন। 🙃

Those patient who are already under insulin therapy(basal), When we start bolus insulin in them ,is 4unit/0.1u per kg/10% of basal insulin for the WHOLE DAY or before each meal?
