    I am seeing post-it notes on top of a wrapped item or on the label of a wrapped it and even on a rigid container. (The post-it will have a room # on it.) That doesn’t seem like a good idea/good practice. Would a surveyor have an issue with this?

    I agree with 85-90% of what you are saying. My only concern is with an associates degree, colleges tend to require too many courses that have nothing to do with the actual career, raising the cost of the education for profit. I do, however, think there should be more tech schools available nationwide. There should also be a nationwide pay standard set to incetivize education. I started 6 months ago without a degree or certification, it is possible to learn on the job but I agree that there needs to be more focus on the job description and the "why" up front. I do tons of research before I start something (there are many resources thanks to channels like yours and Boston career institute) so that helped me pick up on things faster than many of the people I work with. Some have worked there for over a year and are asking me questions. For my evaluation, they only focused on faster processing of instruments over quality. My immediate supervisor tells me that stained instruments are inevitable and not to focus on removing all stains. Thank you for what you do on this channel!

      There are some Surgical Tech programs that participants graduate with AA in Health Science. The program I proposed to my organization consisted of pre reqs. A&P, Microbiology, Med term, etc. Followed by the Penn Foster two-month program, 120-day clinical rotation, Certification, and finally a 120-day probation for graduation.

      This is true, and it's certainly a valid point that an AA degree does have "pre-reqs" that aren't applicable. The pay should absolutely be increased! Unfortunately, the medical system still treats SPD as an "entry-level" position, and they don't want to recognize that there is a tremendous amount of scientific knowledge and technical expertise required to do the job effectively.

    This is such an interesting and awesome topic! Thank you! I just graduated from a 4months sterile processing program and did a 6 week 40 hours a week in a hospital. Sterile technician definitely do a lot. Some people think we are just washing dirty instruments but no. I was amazed by the people who work in the spd. I learned how to do decontaminate different surgical instruments and those heavy loaned instruments, learn how to operate the sterilizers and making sure the parameters are met and making sure the biologicals passed. And I also learned how to picked cases. And so much more!

    UV inspection light... in addition to cement chunks lighting up, you'll see cement "dust" too. Osteotomes and Freers are sharp and not intended to be used as a cleaning device, don' t be that tech. There is Orthopedic and Dental Cement remover out there, check the IFU and see if they're compatible. Don't allow yourself to be placed in an unsafe condition to make up for the incompetence of the OR (lack of POUC)

      If you don't have decision making or purchasing authority in your facility, then you are limited in what you can do. You might try looking into the IFU of the instrument and find where it states to keep the instruments in the open position during the soaking and cleaning part of the process. Then share your findings with the department manager to support your request.

      Great question! ANSI/AAMI ST79 recommends that we test our ultrasonic cleaner daily for cavitation. It also states that we should be monitoring "critical parameters" so we can evaluate the performance of our cleaning equipment. To answer your question, yes. SonoCheck is used to test specifically for cavitation. Soil removal tests like TOSI, tell you if multiple parameters like cavitation, detergent dosing, length of time of the cycle, and temperature of the water are all working together to remove surgical soils effectively.     How does Joint Commission or ACHC expect to see open case being transported to decontamination?

      Great question! Open case carts are permissible as long as soiled instrumentation is contained in a puncture resistant, leak proof on the sides and bottom container with a lid. This is an OSHA requirement.

As far as the units transporting their soiled to items to decontamination… are they expected to display a “dirty” / “biohazard” sticker / signage on their puncture proof containers?

      In ANSI/AAMI ST79:2017, it is recommended to place one or more internal CI in a package so that one of them is visible to the person opening the package, one of them is in an area that is considered least accessible to steam penetration, and in accordance with all applicable IFUs.

      We would recommend that you consult the manufacturer's IFU for the BI product you are using. Some of them have a spot to write the load number, and some may have it on the side.
