One more comment is to use more the MMV mode of ventilation as i can see many VN500 ventilators are available and i think it is a good mode of ventilation if we can use it properly

And to add for the question about ventilation of air leak syndrome HFV will be reasonable as the turbulent flow of HFV will inflate the collapsed part without inducing fistula at the site of leak while the conventional ventilation linear flow will not inflate the lungs as it goes with the least resistance so might induce fistula

Speaking about the best mode of ventilation as AC with volume guarantee it will give the baby the same tidal volume each breath and can kill the diaphragm activity in contrast to SIMV plus VG which give variable volume to keep the diaphragm alert and this is called stochastic resonance

السلام عليكم ورحمة الله وبركاته .very good topics but can added ventilator induce Diaphragmatic injury to the list of complications of Ventilation and that can cause weaning failure .Data from who survived I 22 to 24 weeks gestational age they use lowest tidal volume .thanks
