It works especially well for those patients who, for some reason, are unable to answer the question, "How much pain are you experiencing?" Patients who have neurological problems or who can't answer medical questions. This device, which captures signals and electrical disturbances on the patient's skin, on a finger, without any intrusion, is the only way to know how much pain they will feel in these cases.

How would lidocaine 1 mg/pound prior to laryngoscopy affect NOL? How about 50 mg IV ketamine 2-3 minutes prior to incision? Are opioids really necessary?

Real time EMG as a secondary BIS trend eliminates the need for a nociception monitor. EMG spikes signal incipient arousal. Arousal precedes nociception. By responding to EMG spikes with additional propofol to drive the spike back to baseline, arousal is prevented. One monitor, not two, is all that's needed.
