Thank you this video was very helpful. I do have a question for you. I’ve recently joined a group that has been urgent care but now wants to be urgent care, primary care, physical therapy and behavioral health. Do we need to set up NPI’ 2 per specialty
We called triwest the agent is unable to locate auth fax request despite sending twice n they are prompting to contact Availity in order to submit authorization.. when tried to submit through availity.. they are having now issues like member not found.. provider not found n so on.. it's too much time consuming without any result
@@Rajneshpandit I have actually heard that from multiple people since the beginning of the year. It sounds as if the execution of this change over was not well planned or implemented. I know some clearinghouses are also not able to submit claims either because claim enrollment is required, however, there are some challenges between the payer and the clearinghouse at this time to do so. I am sorry you are experiencing this. Hopefully it gets better in the coming weeks.

Can you please make a video on QMB/ DSNP plans, where the bill goes out first as primary insurance?
@@AkramAli-sl6iq hi there. These are plans where patients have Medicare as primary and Medicaid as secondary. For DSNP, the primary insurance is with a Medicare Advantage plan through a commercial payer, so the claim needs to be submitted accordingly per their eligibility check. Here is a helpful link about UHc’s DSNP and explains how these plans work in general. www.uhc.com/communityplan/dual-special-needs-plans/what-is-a-dsnp?cid=ps:dsnp-ggl-nb-p-dual-pm-8030209:088cf8f6&WT.mc_id=8030209&gad_source=1&gclid=CjwKCAiA7Y28BhAnEiwAAdOJUNgctWVR7d8LROxusLNEqjhqqj99ZTmgqA75MTPByLBi3VoegYunnRoCvWQQAvD_BwE&gclsrc=aw.ds QMBs go directly to Medicare A or B and then Medicaid pays as secondary. Hi both co insurance are 20% My primary copay is 100 deductible is 3000. Out of pocket is 7000. My secondary insurance copay is 40 deductible is 750. Out of pocket is 4000. Do I have to pay 2 copay when I’m in the hospital? 100+40 . Or only 40? If I need a surgery cost 10000 or 25000. 10000 -3000=7,000 7000*20%=1400 Is 1400 goes to secondary insurance? 1400-750=650 650*20%=130? So I paid 3000+750+130?
Hi there and thank you for watching. Multiple insurances can be very confusing and sometimes the way they process depends on the type of insurance each of them are (i.e. commercial, Medicare or Medicaid plans). If both of yours are commercial plans, then you have to meet your primary insurance deductible before the secondary will generally start to pay anything. The purpose of the secondary insurance is to cover anything that goes to patient responsibility after the insurance pays their portion. Insurances will not pay for anything until you hit your deductible. Patients in your situation may feel having 2 insurances that both have high deductibles may not be financially worth it. I would recommend calling your secondary insurance plan, let them know your benefits of your primary plan and ask them when/how they will start to pay. That will help you know specifically for your plans.
I notice on Tricare West website, they are no longer accepting new contracts. Have you heard anything on when they may open enrollment? Or is the only thing we can do is contract as an out of network provider for now?
@@Smiseter6937 you would need to move forward with out of network enrollment, however, it sounds like they pay the same rate. You would just need to make sure that you follow their out of network protocols when it comes to requirements for authorizations and referrals. They have been closed for quite a while, and I don’t think they give an ETA of when they may open it up again thank you for watching!
They payscale for physicians is at a medicaid rate.

In my experience, Tricare West and East always reimburse using the Medicare fee schedule and fees will fluctuate based on the state the doctor resides just like Medicare rates do. However, I have not been able to find anything stating if or how the fees will change with this new contract. I encourage practices to contact the payer directly with these questions. Enjoy your day.

So there are a couple areas in the U.S. that are “dual eligibility” areas. For example, Northern Virginia. If your service is rendered on the west side of Chain Bridge Rd, the local is Anthem. If your visit was at an office east of Chain Bridge Rd, the local is Carefirst. I believe there is a dual eligibility area in Washington state as well. It is not at all unusual for practices in these areas to have multiple offices- some in the Anthem area and some in the Carefirst area. This means a patient with BCBS who goes to office A has their claim subbed to Anthem and then if they go to office B at another time, it is submitted to Carefirst. Moreover, the 2 BCBS’s have very different fee schedules. Try explaining to a patient that yes, it is correct that your visit to office A was more than the exact same treatment at office B! It is the same with federal policies. Some go to Anthem; some go to Carefirst.

Their changes have reduced access and coverage for mental health. I get so many denied claims now for billing issues that I do not understand. When I look on CMS.gov, the ICD 10 codes and CPT Codes are covered. Any advice because when I call I get: “I can’t tell you how to bill ma’am.”
Hi Jennifer, thanks so much for the video. I am a PA practicing family medicine. Is it safe to assume incident to billing is only applicable to chronic care management and not preventative services like annual wellness checks? Also, would it be applicable to an instance when a mid level makes changes to the care plan for a diagnosis that was previously evaluated by the MD? (Ex. Changing HTN medication due to non efficacious response from initial care plan.) Or is it only applicable when care plan stays status quo without changes? Thanks again!
Hi there and thank you for asking! Wellness visits are best to be billed under the NPP performing the services. Remember incident-to is for professional services only so any in-house labs and tests (i.e ECG) that could be done during the visit would not qualify for incident-to billing. Incident-to is applicable anytime there is a care plan set forth by a doctor so even for acute needs that may need a follow up, not just chronic conditions (although that is when it would be most applicable). Making changes to medications, or adding a medication for an existing care plan/diagnosis would qualify as long as that is part of the intended reason for the follow up. Do you know if a rechargeable battery has to last for 3 year for the equipment to be considered a DME? Or can the main equipment by DME, but the battery can be changed every year or two as it ages?

There are different categories of DME and there are already existing CPT codes for most DME that is covered. I would recommend checking the CMS guidelines on the website to see if they mention anything. I appreciate you supporting my channel.
I qualified for disability a few yrs after I started having seizures, and S. Security let me know I was entitled to healthcare. I saw community doctors for a few yrs using my SS#, and it worked fairly well. Then, one day I showed up to a regular confirmed doctor appointment, and the woman checking me in said a company called Anthem Blue Cross was my provider, and asked for my card. I'd never heard of this company. I've had many more appointments canceled and wasted many, many hrs on the phone, in gvt offices... it's really scary and convoluted.
I’m sorry to hear that and it is super frustrating for patients when they make things so complicated. My guess is that your insurance is now through a Managed Care Organization (MCO) with Anthem. I would call your local Anthem plan by looking for the Customer Service number on the internet. I’m sure if you are registered with them, they will be able to assist. Also, if any of the doctors’ offices have access to the Medicare or Medicaid portal then they should be able to provide you with your Anthem subscriber ID and information too. It should not prevent you from being seen either if they take that insurance. Good luck and thank you for watching. who do you call in each state to get the tele health license?
Providers are the ones licensed in each state so you would need to check with each state’s individual licensing board (i.e medical board for doctors and PA’s, nursing board for NP’s, etc.) to see if they offer this or not. They want you to be certified now. you can get a low paying job with no certificate but if you want 90,000 to 120,000 must be certified
In healthcare you will always find this difference and it depends on what type of job you want, whether you want to work for a large corporation or an independent practice. Private practices, especially those on the smaller to medium size would probably not be able to pay that salary even for a Certified Credentialing Specialist. Although Certification will be important, many employers will also state something to the likes of “or experience equivalent” as actually having done the job is invaluable. Thank you for sharing!

I am in Texas, I have an employer who is using my Medicare no for billing, I am in the process of opening a practice as a NP, I already have NPI for my business, how do I apply for Medicare/Medicaid provider number for my business? 2) How do I find my Medicaid provider number being used by my employer?
@@maryoledibe4618 hi there and thank you for watching. Everything for Medicare is done via the PECOS portal. I have videos about this so please be sure to watch those. Once you enroll through Medicare for your new practice, you will be assigned a different Medicare number for the group, as well as for you as an individual working at that specific group. You may need to call your local Medicaid to find out what your number is, however, many state Medicaid plans do the same thing as Medicare and require you to enroll the new group and affiliate yourself as the provider with the group and they will assign new Medicaid numbers that are specific to that practice as well. If you're already enrolled in Peccos and later on you open a private practice should you do an enrollment with just the practice on peccos. Or should you update the individual account on peccos and add practice?

All states have different situations when it comes to IPAs. I recommend going into Google and searching for IPAs in the area in which you are interested. Then go to each of their individual websites, if you have any, and learn specifically about each one that way. Thank you for watching!
I'm still not through and am so grateful for you and your channel and your content! Your Venn diagram of the Medi/Medi plans is just spot on. As you were mentioning, there's more and more of us (myself included, quite for real) on Medicaid plans than Medicare. And then there's the Medi/Medi folk, who are doubly covered. Hello there! I've just started this video and I know I've been very hit and miss this year-- it's been a wild one in my own career path! I came both to hear what I know will be a thoughtful and relevant discussion about this topic. It's a 'squirrely' one that a lot of practices don't enjoy/don't get real with. I have questions , why insurance pay high if provider par with ipa? Why insurance accept ipa terms and conditions and ready for hig payment instead of existing contract rate ? Insurance scar from ipa group. Ipa threat to insurance If insurance not accept the rate then they move all patients to other insurance ? Please advise
IPAs leverage the power in numbers by taking multiple independent practices and providers and combining them into one for negotiating with insurance companies. In many places, if you are not part of an IPA, then you may not bill or be paid by some payers. California is one state that is heavy in IPAs. Other states they are practically unheard of, so it’s important to know the payers in all areas. Good luck and thank you for watching!
This is like many other industries unfortunately (a prime example is the defense industry). A sector dominated by a few big players or an oligopoly. Some type of legislation is needed to “even the playing field.”

I’m in the very early stages of learning and study and confused how to prepare secondary claims on the CMS 1500 form in the case of Medicare being a primary payer and Medicaid being secondary. How would I go about filling out the secondary claim for Medicaid? This part has been most confusing for me
Usually your billing system will attach the primary payer’s ERA with the claim to the secondary payer. You would have to learn how the specific system you’re using will do that. Most insurances will receive an electronic claim so you would not have to send a HCFA 1500 form anyway. In terms of Medicare as primary, if they are aware there is a secondary insurance, such as Medicaid, then they would automatically forward the claim for you to the secondary payer and you would not have to worry about that. I apologize, but I am not familiar with how to enroll a medical transportation company with CMS. I would recommend calling your local Medicare Administrator (MAC) and ask what is required to do so. They are usually very helpful. Thank you for watching!

Thank you for this video, very helpful. I am learning about primary and secondary insurance and have a question. Hoping to have a baby next year and opted for covering through my employer because it includes some IVF coverage. Now thinking of getting covered under my husband's insurance too as everything prebirth/birth related is 100% covered by them, whereas with my insurance we would still have to pay for 20% of the cost after deducible is met (80/20 rule). Am I right in thinking that if I get a secondary insurance through my husband's employer, they would pick up that remaining 20% of cost which would otherwise be my responsibility to cover? Thank you!
