The North Shore PCN is a partnership between the North Shore Division of Family Practice representing North Shore family physicians, the Vancouver Coastal Health Authority, the Musqueam (xʷməθkʷəy̓əm), Squamish (Sḵwx̱wú7mesh Úxwumixw) and the Tsleil-Waututh (səlilwətaɬ) First Nations, and local community partners, along with other community providers such as nurse practitioners. All partners are working together redesign our primary and community care system so that it works better for patients and health-care providers.
