Informative video Dr Owen. My Sculptra treatment was completely painless. I had no bruising, just a little puffiness post injection that lasted a few days. Massaging was fine and all seems well. Only the sites injected with Kybella are still a little bruised and swollen. Looking forward to the next treatment session. Merry Christmas. Anna

Hi Owen. Very informative video with some new research too. I've been a fan of LED therapy for many years. I have the Aduro Mask, the handheld LightStim device for my scalp, and a largish foling LED panel. LED has always helped my psoriasis in spite of being photosensitive. I also found that an at home IPL device has been really hepful in treating post surgical thread vein clusters and any residual redness remaining from fully healed post surgical incision sites. May I ask if it will be possible to wear a mineral sunscreen and mineral founation powder immediately after treatment on Fiday to help protect and camouflage my photosenstive post treatment skin for the journey home? Did you experience the storm these past 2 days? I immediately assumed Putin had pressed the nuke button when the mobile phone siren went off. 70mph winds battered my little house and garden that is open to the elemennts, and we've had no power since Friday evening. Only only hour ago did the power return and I could finally have a much craved coffee. It's pure heaven. First world problems eh! Looking forward to Friday. Kind regards. Anna

Thank you so much for the informative comparison. But if I may asked one question, you said Radiesse is used outside the face like neck and hands, does it mean radiesse is not meant to use for face? I have droopy cheeks that is causing sagging at the Nasolabial Folds.
Hi, thanks for the question. You can certainly use Radiesse in the face - most commonly as a very lifting filler but also diluted. There’s not much to choose from against Sculptra but there is just a bit more evidence for Sculptra in the face. However Radiesse diluted seems a bit more versatile where you can use it. Both are great products!

Hi doctor! I’m planning on having Radiesse injected in my jawline and chin. Does it hold its shape well? Does it migrate? Does it cause unwanted collagen growth? Also how long should you let it settle before getting other procedures done? Any advice is appreciated, thank you so much!!!
Hi, thanks for the question! Radiesse undiluted is an amazing treatment for the jawline and chin; just make sure your injector is experienced as it isn’t reversible and you have to be really careful in a female not to widen the jawline. When done properly however it is the best filler to sculpt a sharp jawline. The filler is actually 70% a carrier gel and 30% calcium - the gel goes in 12 weeks and the calcium around a year. It rarely migrates in my experience, in part because collagen fibres grow around it - you can never have too much collagen! Once done, 2 weeks is pretty standard between most aesthetic procedures but you should check with your practitioner as it depends on what the other treatment is of course. Hope that helps!

Hi doctor~ Which one would you recommend for nasolabial folds? In my late 20's and my fold is down to my lips.
Amazing question! Personally if the fold is deep I would go for Radiesse as it has a volumising as well as biostimulatory effect. You could even blend with a HA filler which gives a little reversibility. One issue is making sure the injection doesn’t go into the facial artery which often runs along the fold - if possible ultrasound is great for that purpose. Best of luck!
Please doctor hope you see my question please answer me. I had radisse diluted with saline for the jaw line area and the doctor used the cannula and spread the product and use med fanning technique but I find after 4 weeks of the procedure that my jawls is bigger and sag what could be the reason. The doctor said the radiasse is only biostmualtor and not filler so he have nothing to do with radisse what could be the reason please
@@abeerkholghi7128 Hi, sorry to hear you have had problems. It depends on the dilution and area used - it might have been diluted ok, but if less than 2:1 (2 saline for 1 Radiesse) it still can act a little like a filler. If a syringe were used just along the jawline, it might be a bit concentrated. On a positive note, things should improve - the CMC gel which is 70% of Radiesse goes in 6-12 weeks and the calcium over 6-12 months so it should gradually go down, and there will be more collagen long term to support the jawline. Hope everything works out for you.

👋🏻 Hi, the concern is theoretical rather than from studies- it’s the risk of nodule formation. I would certainly consider treating if the disease was stable and well controlled such as on carbimazole or after surgery/radioiodine but not if uncontrolled or symptomatic. Lovely explanation, doctor. I've a question I hope you can answer. I have deep nasolabial folds due to fat that was transferred to my face in 2018. I was considering Raidesse to help lift those folds. Would you recommend that? Thank you :)
I love Radiesse and this would be a good choice if the folds were very deep. My only suggestion would be to have done with ultrasound as the facial artery is very close to the fold and Radiesse is irreversible (a vascular occlusion here with Radiesse would not be good ⚠️). I do live guided injections as you can see the cannula tip at all times.

@@angelamarkopoyloy7546 good question and although no comparative studies Sylfirm is likely to be effective for many of the same benefits. I use Morpheus8 around the eye. The downside is the high cost for a small area of treatment and the discomfort from RF microneedling, but if possible both together are great. Thanks for the interesting comments!
The question is whether all these regenerative treaments stimulate something harmful instead of collagen!!In the USA you can only microneedle polynucloetides!You cannot inject...So why then?
@@angelamarkopoyloy7546 What is it you think they might stimulate? We know they reduce oxidative compounds like MMPs that cause harm, so I don’t see what is going to be harmful. I can’t comment specifically on rules in the US, but the regulations tend to be very (and often overly) strict with most injectables due to various historic factors. Ultimately if it’s safe to microneedle into the skin, as long as a recommended dose there’s no reason not to inject in my opinion.

In my experience it is fine - the way Radiesse works it doesn’t stimulate the immune system but acts as a physical scaffold and pressure stimulates fibroblasts so it should be fine

@@Cheers12324 While it’s only my opinion and not tested in studies I think this would be fine - all left at that point is calcium microspheres and no know interaction with PLLA so I think would be absolutely fine 👍🏻

Hi Karen, I dont use PCL products - do you know which one was used? It's common with HA products to have a blue hue but black is more puzzling. Significant bruising can persist in the under eye area for a few months, but less likely to be both-sided. If you know the product I will have a look for you and see if I can suggest something

Hi yes it will, typically the CMC goes within 12 weeks and the calcium microspheres in 12 months
This feels like it was quite heavily focused on pigmented dark circles. For those of us with blood vessel dark circles, what is the best route, and are there any at home treatments that will have any significant effects?
Thanks for the feedback and sorry it didn't address as much as hoped. In clinic I go for polynucleotide with RF microneedling, although with dermal filler as well if a deeper groove. For home treatment as well as good sleep and diet, I would suggest retinol creams, starting low dose and building up gradually. One thing I didnt mention is red light - while it may not eliminate vascular dark circles entirely, regular use can help improve skin thickness and reduce the prominence of blood vessels. I would suggest looking for at-home LED devices that have a wavelength of 600-650 nm for red light therapy. I am doing a forthcoming video on red light. Hope thats helps and thanks again

Yes it’s typically 2-4 weeks before resuming normal activities and many months for all the symptoms to subside

Wow, great explanation! I’m definitely following you. I have a quick question: I’m planning to get Sculptra in my temples, since you mentioned that Radiesse would not make sense in that area. However, I’m concerned about it spreading to the area under my eyes, which I’ve heard can happen often. Do you think filler would be a safer option? Also, what color is Sculptra? Is it white like Radiesse?
Hi Madeleine great questions! Sculptra is commonly injected into the temple and this is usually under the superficial fatty layer just underneath the skin where it has more effect on the overlying skin. It’s possible that some particles are going to move with gravity downwards into the fat around the outer eye, but this shouldn’t necessarily be a bad thing this helps with the skin around this area which is really thin so it can slow down the crepey under eye appearance. Filler is a good choice with more significant volume loss and hollowing in the temples but it does tend to look quite puffy initially, sometimes for a week or two. I use ultrasound live injections with the temple for safety and accuracy. Sculptrais actually white in colour as a powder but when you make it up into a suspension you can hardly see it where Radiesse is pure white even when blended with saline so it is much “whiter” than Sculptra in appearance. Hope this is ok!

Doc what can we do with migrated very old restylane filler that came back to make an appearance on ones face ? My doc tried twice to dissolve, no luck 😢. Anything natural?
thanks for the question, if it is Juvederm it is going to be hyaluronic acid. The only way of getting rid of this more quickly is with the dissolving enzyme and unfortunately there’s nothing natural that will make any difference that I know of. If it’s very old filler sometimes this can become compressed into little pockets and the enzyme doesn’t dissolve very well if it’s injected without any imaging guidance. I’d suggest seeing a doctor who specialises in ultrasound guided dissolving; I do quite a bit of this in my practice in London. This should target the small pockets accurately and also uses a lot less of the dissolving enzyme which minimise the chance of allergic reaction. I hope this helps!

Thanks for responding. Sadly I live in the US It was old restylane 😢. I just had fractional co2 laser, full face and it is making it a bit flatter. I’m 4 months out ❤. Hope it gets better if no ill seek out a doc that can do what you do, the guided method

@@Saibu-uo4ts Hi, the carrier gel CMC goes in around 6-12 weeks after injections - that’s 70% injections volume. However collagen production increases from around month 2 to 1 year - if it’s collagen then it will gradually reduce over several years. When did you have the treatment and how many syringes?
@infinityaesthetics Thanks a lot for the answer! 🥲🙏🙏🙏 He said it was in total 1ml of 1:1 diluted (with saline) Radiesse in my cheeks (in subcutaneous plane) Treatment was in April 2023...so basically 17 months after. It was paired with subcison though (for an acne scar treatment). Cheeks just look very chubby, esp when smiling and I never had that and weigh less now then before treatment:( How many years you guess. There's literally no study on actual longlivety sadly and on whether it EVER returns to baseline :( wouldn't it just stay permanently cause it's your own collagen now? Or at least it won't go away quicker than your natural collagen right? So the distortion would be there permanently....? Thanks a lot for sharing your knowledge. Appreciate it a lot!

That’s completely unproven and extensive evidence suggests otherwise. 1. Safety Profile and Carcinogenicity Assessments • A comprehensive review in Dermatologic Surgery evaluated the safety of injectable fillers, including their carcinogenic potential. The study concluded that no direct evidence links HA, CaHA, PLLA, or PMMA fillers with cancer, highlighting that these fillers have been extensively used without reports of tumour formation or cancer induction (Goldberg et al., 2020). 2. Case Reports and Long-term Follow-ups • Long-term Safety Review: A 10-year safety review of hyaluronic acid fillers published in Plastic and Reconstructive Surgery found no reported cases of malignancies directly attributable to filler use. The study’s extensive review of adverse events did not identify cancer as a complication (Bertossi et al., 2021). • Granuloma Formation: While granulomas are a recognised adverse effect, especially with synthetic fillers, these are benign and do not exhibit malignant transformation. Studies, including one in Journal of Cosmetic and Laser Therapy, found that granulomas were reactive, not neoplastic (Urdiales-Gálvez et al., 2018). 3. Preclinical and In Vitro Studies • Animal Studies: Preclinical studies using rodent models to assess the carcinogenic potential of PLLA, PMMA, and CaHA fillers have not demonstrated tumourigenesis. For instance, a study published in Toxicologic Pathology examined long-term implant sites and found no evidence of neoplastic changes (Brandt et al., 2022). • Cellular Assays: In vitro assays designed to assess the mutagenicity of filler components consistently show no mutagenic or carcinogenic activity. For instance, HA fillers were shown to have no impact on DNA integrity or cellular proliferation beyond normal wound healing pathways (Jansen et al., 2019). Regulatory Position and Post-Marketing Surveillance Regulatory bodies such as the FDA (U.S.) and MHRA (UK) continuously monitor the safety profiles of dermal fillers through post-marketing surveillance. As of the latest updates, neither the FDA nor MHRA has issued warnings or identified cancer risks associated with any approved dermal fillers. These findings align with clinical data suggesting that when used according to guidelines, dermal fillers do not pose a carcinogenic risk. My opinion therefore is that in the current evidence, there is no substantiated link between dermal fillers and cancer. The materials used in fillers-particularly hyaluronic acid, calcium hydroxylapatite, and other biocompatible substances-are considered safe and have not demonstrated carcinogenic effects in clinical or preclinical studies. The main risks associated with fillers are typically localised reactions such as inflammation, granulomas, or infection, which do not equate to systemic carcinogenicity.
@@infinityaesthetics recent review found that the number of skin cancers in tattooed skin was low, and therefore seems coincidental, however a number of carcinogens that have been found in tattoo inks have been associated with cancers elsewhere in the body, such as the liver or bladder.

Something to consider in your presentation style for future videos: I would like to encourage you to refer to ‘ladies’ simply as women, especially when you’re discussing how various treatments affect men or women (who may or may not be ladies). I’m not sure why this difference in terminology persists to this day but I find it a bit irritating. It seems men have no trouble referring to each other as men but oddly, (many, not all)men have difficulty refraining from referring to women with various diminutive names such as ladies, gals, girls.

I think you are referring to the TikTok post about filler on MRI - this is thought provoking but isn’t high quality evidence with one patient where there may be confounding factors. We know from recent MRI studies that most patients have filler persisting for 2 years but this can occasionally be up to 10-15 years. Many areas usually resolve fully within 2 years. Persistence isn’t necessarily a bad thing if done properly and not refilled frequently. Most filler treatments I do don’t need to be repeated for more than 2 years and often longer - I agree if done regularly it can certainly cause oedema and ‘pillow face’. It’s all about being cautious! Thanks for the very interesting question!
