    Mine was blocked in almost that same place and it took 2 stents to fully open it. The middle one 60% blocked, 1 stent. The third 40% blocked left alone. Didn't feel any difference to tiredness after the stents, but my heart showed no residual damage.

    Is it possible that if a sent is placed in a perfectly healthy and clear artery that it could collapse and migrate 3 days later? In layman’s terms, it seems that there would not be any “glue” for the stent to stick to / be imbedded in and it seems plausible that a stent would collapse in a perfectly healthy artery. After collapse report says “Heart cath found dislodgement of stent, now lying partly in the left main and partly at the ostium and proximal LAD”

      Very very rarely, if at all. Stent collapse are generally an acute phenomenon that are usually observed in the cath lab itself. Rarely (but not limited to these clinical scenarios) a stent may land in a Myocardial bridge or a very calcific portion of the Artery, and may lead to underexpansion or even stent crushing/collapse which may present as a collapsed stent. Moving is extremely rare. I'd do an OCT/IVUS before labeling it such. However, it is a very rare occurrence and I'm quite sure, there might even be another reason to it, depending on the scenario or the clinical context text, which presently, I am not very well aware of.       If possible, after consulting your primary doctor, get a Xray Cervical Spine - AP and Lateral View done. It is not a very expensive test.

      Myocardial bridge itself causes a compression of the Artery during systole, mimicking a type of Stenosis during systole, relieved during diastole by itself. The question more apt would be, can atherosclerosis/plaque Rupture happen in a Myocardial bridge- yes, it can, specially at the proximal or the distal edge. Hence antiplatelets are usually recommended in a case of Myocardial bridge and also to aggressively control all risk factors.

Whether it requires a bypass or not- depends also on the location of the block, comorbidity and number of arteries involved. Hence, the information is a little insufficient for me to actually comment on whether bypass is required or angioplasty. Hence, kindly give more details on the blocks location, estimated length of the blocks, number of vessels involved, comorbidity, age, and EF. Thanks and have a great day ahead sir.

      Heart disease and disease of lungs can go hand in hand and often mimic each other, or sometimes even co-exist. Heart disease can still be there, without blockages- for example as in case of valvular heart disease, or endothelial dysfunction or Myocardial bridge or congenital cardiac anomalies amongst others. Hence, I would suggest a comprehensive examination to rule everything out and not just blockages. Have a great day ahead, and hope you remain in the best of your health always.

To answer your question, whether 2.5×38mm is counted as a large stent or not- the answer is quite simple - large stent or not does not matter, the stent put is matched according to the lesion length and the natural artery size to give the best results possible.       Hello, Good morning. Hope you're doing well and may you remain in good health always. See, the thing is - even cardiologists wish that number of stents implanted be less, however, many times, the anatomy, the length of lesions, or different arteries are involved- which necessitate more number of stents. If your doctor thought that 1 stent will not be able to give the desired result, then he/she may have out additional stents. Less number of stents is always desirable, however, having said that, most of the times, situations demand more number of stents. Hence, I think nothing wrong is done, it is just that, your artery anatomy/ lesion length / number of vessels involved may have necessitated more number of stents. Don't leave medications without consultation of doctor. And don't smoke/don't consume alcohol, tobacco and pan masala. Hope you remain in great health always sir.

      On case to case basis. Healthcare still has to consider the cost of doing something. We do have IVUS, but we can only do them in cases where they can afford it. It is a desirable thing.

    Amazing work sir...however, whats ur take on medical mx for such patients? Intracoronary tirofiban, and triple therapy for a month later..?

      Well thank you for the question Dr. Medical Management alone for such patients is futile in my opinion. I am guessing, we are discussing about the presence of the thrombus scenario in a patient. The reason is that in this particular case- the thrombus is well organized and "hard" (not exactly 'soft' as you might put it. Such type of thrombus do poorly with medical management alone and take a lot of time to clear, if they clear at all, that is. In a patient with ongoing chest pain, and no distal flow, with a not-soft thrombus, there is limited scope of medical treatment alone, and patient would require PCI asap, like in this case, to protect the myocardium as much as possible. About intracoronary tirofiban, I like tirofiban, and some use cangrelor, well, it all depends on a lot of factors. About the triple therapy, in this case, the patient was only kept on DAPT after adequate treatment with PCI. Triple therapy may theoretically sound better, but it subjects the patient to a massive amount of risk for bleeding. But yes, if the thrombus was soft, and the flow was non-limiting and patient pain free at the time, i might have gone with the approach that you were talking about. But in this case. The only right option was this one due to all the factors mentioned above.

      Of course. That's a key point. Dislodgement of aspirated thrombus can be catastrophic.

      Stents can be removed surgically, but it is a risky procedure. Most of the time, after sometime, stents get embedded within the arteries structure and become a part of the Artery hence usually removal of stents is not required. Stents are put when there are blockages to allow passage of blood unhindered and preventing collapse of the Artery. Sometimes, stent may be required to be removed, if needed, in case of any infection that somehow reaches the Stent Or there is reaction to the Stent, which can only be found out after putting a Stent and not before. It is a very very rare event, but sometimes still happens. It is a risk that most of the time has to be taken. But still Stent removal is not a common thing to do.

      Recommended in the literature provided with the product. Mostly because it is a very soft material and very prone to kinking.
