If by the time you are playing the lottery the child is under 21 e.g 20 yrs 11 months you can play with her, and if the results comes out and the child has turn 21 and still unmarried you can add her ds260 but if the child turns 21 and has a put to birth the child can travel but the grand child can't, lottery is for father, mother, biological children and legally adopted children. Though you can file for siblings while in the US already

About the polio vaccine you need not worry, on the day of your medical appointment you just have to take your childhood vaccination record till date of you have but if you don't have any vacation record once you go for your medical exam they will make sure you receive all the necessary vaccines my dear.
