I got one literally the exact same, carving and all, in Medjugorje in BH 4 years ago and I could never find any information on how to play it until now. Thank you for the help!

In my grandmas village there lives a man who loves to walk through the village with his dogs and he always brings his dvojnice along, its a beautiful sound but a bit creepy when you hear it at 2 in the morning

This is very common in Serbian people! My grandpa used to play this a lot along with Gusle and Gajde

I bought one very similar in a yard sale in the mid 70s. The left side doesn't play. I believe it is occluded with dust or the reed is warped. I've had it since I was very young. Are the designs decorative or do they have meaning?

I own a bunch of different folk flutes from different cultures and one of them is this dvoinice(a). To me the way the notes come out sounding very dissonant sounds exactly like that one instrument that is from the Balkans called Gusle. I mean the one string instrument that is played with a bow and at the end of whose neck there is usually a carving shaped like a goat with big horns, or a horse. How can I put it politely. One string Gusle is of course historically important, but it does not sound pleasant. When I try playing dvoinice most notes come out exactly like the notes from musicians on TH-cam who play Gusle.

Yeah... That's the sound I remember. My Serb grate grandpa would always have one with him, playing it all day long. Was bed bound in his last yrs, didn't want a tv in his room, and guess playing was the one thing that kept him sane. Had both double and single one next to him. But he kept one with him troughout his whole life, pissing grate grandma off, since you could always hear it :) Grandma was like "we kids would wait to hear his flute from far away, going down the road, coming home from work, and would start heating up lunch". Guess it can be played pretty loud :) In ww2 it was good if you knew how to play those smaller instruments, that could be made everywhere

Perhaps it worths to note that the instrument you are playing is traditional of Sardinia and Sicily islands from thousands years. In fact it was known yet by Etruscan and Romans.

The percussive part is the best.
