    There appears to be lengthening PR intervals and then a dropped beat, is this not more consistent with Mobitz type I?

    Folks in the UK a please note use Hb units now of g/L so the 6 g/dL is now referred to as 60 g/L

    Hi great session, but can I ask why there wasn’t tachycardia with the assumed blood loss?

      Hi there great question. Sometimes patients surprise us-and don’t behave as we think they “should “! Young people, sometimes the elderly (who may find it harder to “mount a tachycardia “) and patients on beta-blockers, as well as those who have lost only moderate amounts of blood or who are under the influence of drugs that might slightly slow the heart (such as anaesthetic drugs) may not have a tachycardia. That said you are totally right; a conventional response to hypovolaemia would include a tachycardia after a while if enough blood is lost. The patient will increasingly vasoconstrict. Don’tforget MAP = CO x SVR

    But given the most likely diagnosis would an ECG add much? Whereas a CTPA would be much more helpful

      Brian your right in many ways. I'm really using these SBAs' to teach the students- not as mock exams but as vehicles where we can chat about the options when they're first learning about clinical medicine. When I record them again I might change the question to something like.."what test are you going to do in the first 10 minutes' or similar..

      Hi you're totally right- and ones like this one wouldn't get into the real exams! It's quite hard to make all the materials myself though. Also I made them in the spirit of teaching - it's just another way the students (they see them at UCL in year 1 clinical) can learn things.
