I'm just throwing my guess here, but I think it's because your Railjack and/or crew got destroyed. Normally they become invulnerable or the enemy ships/defenses stop attacking them when you enter the main objective.. But I think a ramsled or something can get through if they penetrate the same time you enter their ship.

I actually really liked fire mines. theres just one little problem when theres a bug breach and i put the mines. My teamates run directly in the direction of the bug breach. (this is not just low levels by the way its also like 50s to 100s)

Had a similar thing with both Sister and Kuva Lich. While the Sister ones failed the mission, the Lich didn't make you fail. It just makes you pop out to the space and you can fly back/teleport back to the Railjack with mission complete. Idk if it has anything to do with this but when you beat your Lich, you can teleport back immediately to your RJ while beating your sister 😏 you have to go to the Extraction before teleporting back.

Yep, had a similar thing happen to me, I suggest it's because the execution animation puts you into some other object, either map geometry or another enemy hitbox and so it just defaults you into archwing which immediately fails the mission
