How can what Ivy did be called cheating? It's the same thing when people are counting cards playing black jack, then it's for some strange reason called cheating, but how can it be illegal to use your brain? It's illegal because the house is always supposed to win, and when you use your brain and beat them, you will be suid or thrown in jail.

The Phil Ivey one sounds like a them problem, although he did ask for the deck which kind of makes it a him problem, I guess, that’s a tough one

The Phil Ivey incident is total crap. He made demands that were met. If a known professional card player told me that he'd play at my house but only if he could us a certain deck, I'd know there MUST be something special about it. He didn't take his own decks or manipulate the cards in any way. Why should the casinos not have to pay him?
