I love AI scribes. However when getting down to first principles we should think why chart in the first place. Is it for patients or for billing or for medical legal or for our colleagues? From 30 years ago to today notes/charts continuing to explode in length and often include not useful information (think of the common 16 page discharge summaries we all get now). Probably 90% of what we write isn’t useful to patient outcomes. I hope over the next years AI scribes will really move from note taking to truly being a physician and patient assistant working before, during and after the patient/physician interaction to triage, help direct histories/physical exam and investigations/ and ensuring appropriate follow up and patient education. Patients feel they need a single GP for continuity but often this is really based on the single GP having the patients records. It’s not possible for a physician to remember 2000 patients CPPs so we all depend on our charts. Once AI assistants are standardized and patients have access to their own personalized AI assistant - patients won’t need a single GP anymore but can be shared across a team of providers appropriately.

I do think we should revisit even the word AI Scribe. A scribe just copies or paraphrases what is said - it will replace charting - but why do we chart? Getting down to why we even write notes is interesting compared to 60 years ago when it was pen and paper. Will scribing help patients or just add to more non useful information in a patient’s chart like has happened in hospital with new EMRs. Just think of how long discharge summaries have become. Thinking about how AI can be an assistant and action items based on the patient and presentation will really be the revolution in medicine. Ai triaging the patients issue and deciding on which person they need to see will be great and help offload doctors from dealing with issues out of our scope and best dealt with by other healthcare professionals. Great point about revisiting the term "AI Scribe." As you mentioned, the traditional role of a scribe is to copy or paraphrase, but the real value lies in understanding why we chart in the first place. If AI can help reduce unnecessary information in patient charts-like the lengthy discharge summaries we see today-it could be a game changer. The true revolution will come when AI acts as an assistant, triaging patient issues and directing them to the right care provider. This would not only improve efficiency but also offload tasks from doctors, allowing them to focus on what they do best.
This is excactly what I have beend discussing with some people but sadly in Austria noone seems to understand that yet. The individualised approach would be the best, not just in medicine but in pretty much most of areas (education, jobs, how to handle crime etc).,. I really hope that in the future it will be a society where people dont get measured with others but with themselves and shaped accrding to their potential.

It’s unfortunate that it’s not fully recognized yet in Austria, but hopefully, in the future, we’ll see a shift toward measuring people by their own potential, rather than comparing them to others. How competitive the hourly rate? Currently, it is 500 for three hrs locums in ON... Why not doing part-time Reserve as a medical office in CAF instead?
While I was speaking to Dr I offline after the recording it sounded like the rates are competitive with what you listed or even higher. I think it varied depending on the contract/location and he also mentioned that since it is a private contract (rather than being a hard fixed number like when billing through the province) you do have some flexibility to negotiate so there's a little bit of an element of the art of the deal when it comes to signing up. I can't recall about the insurance or pension but Myriam Deroo (one of the HR leads at Calian) would be the best person to ask! Myriam.deroo@calian.com
