Hi, I just got the notification for your comment! I got the melody/lyrics out of the Worship and Song hymnal (#3149), which is from the United Methodist Church. But, I came up with the arrangement myself (accompaniment style, some of the chord progressions, etc.). I’m afraid I did that more in an improvisatory way & didn’t write anything out, otherwise I’d gladly send it to you!

Wonderful. .... The words were written 1834 by Edward Mote . He was pastor of Rehoboth Baptist chapel, Horsham England . The chapel is still there today , preaching Jesus Christ is Lord
