It's a well known thing, there is a category for speedrunning by the name of Max. Casualties that it's required kill that mudokon. It's a requirement even for the subcategory that doesn't allow mayor glitches. Also in Exoddus you can kill an extra mudokon, visiting the screen where your sick companions are to load entities and then using a Flying Slig to spam bombs in the adjacent hole to that room. The first one in line will be killed. Using mayor glitches allow players to kill extra mudokons in both games, it's just too crazy. Some inputs and mechanics to accomplish this are a pain in the butt.
