The only issue is that Lidocaine doesn't have the best bone penetrating capability, and therefore you occasionally get pts that won't get completely numb. In these cases it is best to use Articaine 4% 1:200,000 epi (or 1:100,000 if you need more hemostasis) or even 4% Prilocaine 1:200,000 because they are better at getting in there and eliminating those "hot spots".
