Wow ! I'm not even from america or from any english up bringing speaking countries but I do know that to have a "monkey on his back" in english means to have a drug addiction ! I've heard somewhere long ago that this song was written by the guy who wrote the book 'Leaving Las Vegas". Don't know if that's true but it sounds like it could very well have been so !..
