You did not provide an EF for cases 1, 2, and 3. Also, case 4 looks like a cardiomyopathy and the EF does not appear normal or near normal, also EF is not 55%.

Mechanical Valves are garbage, the patient has to be on blood thinners for the rest of his or her life. Also, TAVR valves are a death warrant, my dear mom's health declined right after she had an aortic TAVR valve put in. My dear mom didn't deserve the progressive health problems that ultimately ended up with sepsis that I truly believe the valve was infected when it was installed in her.

Quite interesting case, echo in IVC as a complication in AMI setting could be quite challenging, do not expect a linear defect, particularly in RCA territory AMI.
