What are the Different Stages of Breast Reconstruction?
i feel so stupid with all tummy
I just had part 1 of my lap flap reconstruction 14 yrs after my 1st diagnosis and 6 yrs after my 2nd diagnosis. Best decision I’ve ever made
I have a very small 9 mm tumor, invasive ductal carcinoma, 2 o'clock position, HER2 negative, 90% ER, genetic test checked 79 genes and results showed no mutations, there's no history of cancer in my family. I'm having surgery in 3 weeks and was thinking of asking my surgeon to skip SLNB. Clinical exams, US, and mammogram didn't detect anything on my lymph nodes. Your thoughts about this would be much appreciated. 🌷🙂
I plan to have a diep flap after masectomy on right side and after having 4 kids my abdomen is bigger than normal. I’ve have facial palsy after a brain tumour which is permanent, now I’m loosing a breast. Having this diep flap is something I’m actually hoping to have but also loosing the weight around my belly and having it tighted is mentally helping me through this physical loss. Being a woman and loosing so much of my confidence through all these physical changes first loosing my face and now my breast has been so mentally hard for me but knowing I could get a body transformation which could boost my confidence in the fire is what is keeping me going and fighting. I would like to know would the NHS in the uk cover this? The muscles on my abdomen have split apart after babies and I would like these repaired at the same time, would the nhs cover this at the same time as the diep. If I’m gona get the diep I would rather get a whole transformation and feel mentally better about my body. Please help
Thank you for this video. I will be doing my reconstruction in the next 2 months
You are so welcome! You’ve got this!
After Diep Flap, my abdominal area is bulging, looks very uncomfortable and ugly. I’ve never had bulging abdominal, I was always flat. It bothers me. Please tell me can that be fixed?

Hi Alisa. I'm very sorry to hear this. If this has not improved over time, or even gotten worse, please speak to your surgeon. This is something that can generally be addressed but unfortunately more surgery is often involved in the form of tightening of tissues internally, or reinforcement with mesh. Your team will need toe evaluate you and also make sure you do not have hernia. If you need to, get a second opinion.
I’m a year post op and still dealing with the infamous Ken doll and swelly belly. My PS told me to do core exercises for the Ken Doll. Will this really help it go away?
Bedankt voor het delen van deze video en de nuttige informatie.
thanks for the info
You’re right, women don’t ask much about the aesthetic details about this surgery. I didn’t and now I’m hindsight wish I had) . I had my surgery last October, and even though I don’t regret the surgery and the results (neat clean incisions and surprisingly pain free post op recovery), I wish I had asked more questions about the aesthetics. My concern is the unsightly sides which looks like horns on each end of the incision along my abdomen.
Good news... your surgeon will likely be able tot help you with those. The excess tissue the you're referring to on each end of your diep flap scar are affectionately known as "dog ears". Scar revision surgery is a simple, low-risk surgery than can remove the excess tissue and create an even, smooth contour. It can make your diep flap scar a little longer but its a trade-off most patients are very willing to make. Talk to your surgeon about this. They should be able to address this for you.
It’s a MASTECtomy.. 😅
I have expander for 11 months now I have my 1st reconstruction in June but I was wondering if it’s normal that my tight breast tenses up lately with pain and a lot of discomfort around the implant above it and my side .. I’ve been feeling a bit stressed lately and I feel my stress goes to my boob and it creates this tension in the area idk if this is normal if it’s just me but I don’t like it. Could it be normal pain? Thank you for your videos!
Your pain is probably from capsular contracture. This is build up of scar tissue around the tissue expander which can cause a change in the breast’s appearance, discomfort, and even chronic pain. While capsular contracture is a fairly common complication with breast implants, it tends to be much more aggressive and problematic in ladies who have undergone radiation therapy to the chest, either as part of their breast cancer treatment, or at some point in the past. Your symptoms will hopefully improve once your tissue expander is replaced with a permanent implant. Some surgeons combine this with fat grafting. Long term, if you continue to have issues with capsular contracture and pain, the definitive treatment is usually to remove the implants and the surrounding scar tissue, and replace them with your own tissue (ie flap surgery). There are many flap options to consider depending on your body habitus and preferences. You may be a candidate to proceed with flap surgery now instead of implants if that makes more sense to you and your current situation. For more info please check out prma-enhance.com and also download the Breast Advocate App free at breastadvocateapp.com.
when you say the multiple abdominal surgeries , I have had 2 sections and I have had keyhole surgery twice? would the keyhole make massive differences?
No, “keyhole” procedures (laparoscopic or robotic) do not make a massive difference.
Praying for a healing for everyone amen try cannabis oil and THC and worm medicine for animals to kill cancer amen
Misleading to show nipples as post mastectomy most women won't have nipples
This animation shows a nipple-sparing mastectomy. Nipple-sparing mastectomy (NSM) is the latest evolution in mastectomy technique. The procedure preserves the entire skin envelope and nipple-areola. Only the underlying breast tissue is removed. More info here: prod.breastadvocateapp.com/contents/232
This was great to hear, thank you for all this information. I am scheduled for a double mastectomy January (happy I was able to get it pushed out until then, although doc is not too crazy about the delay; however, having said that, they will be doing the magtrace with a couple of lymphs removed at the time to doing a sentinel at conclusion of that surgery if the stat path comes back positive. Because of this vid, I did write the surgery RN just now and explained my concerns and to postpone that part of the surgery until I have all the information after the surgery, so we'll see how they take my request. I hope they understand my concern about this topic and it won't be an issue or me stepping on anyone's toes. But, I truly appreciate knowing this upfront and I would be okay not doing a sentinel biopsy even if the 1st couple come back positive... I'd rather hit that head on and deal with my decision rather than regretting it later when it could have been avoided.
Im flat and i love the way i look ❤
Don’t cut them off
Pain to feel after removing your tits for no reason 1:46
Fanthom pain from no breasts
No pain there is a 0:18 alot of pain
This is why I love Dr. C. Is because he gets that the patients are as much a part of the decision and knowledge base as the doctors. Sadly for years and most of the time patients are afraid to advocate for themselves. Go Dr. C.
Where is this Dr located?
I was diagnosed with breast cancer 5 years ago. I had my left breast removed first with an expander placed. Approximately 8 weeks later my right breast removed and diep flap done. My surgeon was phenomenal and my surgery was a success. My scar (which is almost non existent) is just above my pubic bone. My breast reconstruction as well is awesome! Hats off to Dr. Koltz!!!! Hope e made a scary diagnosis much easier to bear!!
How about when the patient has Lupus Induced buy the Chemo? What are the risks?
Women want symmetry, a nice profile, no pain or discomfort and no extra skin, lumps or bumps after a mastectomy.
Very good video I’m about to have my DIEP FLAP surgery and you have brought so much knowledge to me. I want to thank you.
WHAT HAPPENS WHEN A FLAP FAILS ?
Ok I went for a follow Up after 3 weeks of my diep flap. Wasn’t checked for flow? Should I ask why?
Is fat necrosis common 6 weeks after diep flap? I can actually feel the flap above my breast and it feels a bit solid..
So grateful for this resource. Learning about the benefits of delayed reconstruction makes me feel better about my own journey and how things have played out for me.
DIEP flap is not worth the damange that is done to the abdomen. do not do it.. please..your body will change in ways that you cannot image. you will become almost suicidal
i was left with a pucker on the side of my scar that shows through my clothes. At times you are in such a fog after the diagnosis and you don't always ask the right questions. Also it has been almost 20 years since my surgery and i am feeling discomfort and tightness in the area around the breast.
I was told if u use your own tummy fat, there is a 30% reabsorption so saggy boobs n also when lose weight it’ll get alllllll absorbed so now u have only a a or a cup n then become saggy w these two things. I intend to lose a lot of weight too
I WISH SOMEONE WOULD COVER THE LESS OFTEN OCCURRING SIDE EFFECTS OF RADIATION THERAPY IN OVER WEIGHT WOMEN. MORE AND MORE NORTH AMERICANS ARE BECOMING OBESE NOW SO THESE SIDE EFFECTS LIKE SEVERE RADIATION FIBROSIS AND VERY LARGE CONTRACTURES OF THE BREAST AND PECTORAL MUSCLE AS WELL AS THE INTERCOSTAL MUSCLES. THESE SIDE EFFECTS ARE DEVESTATING. ALSO MOST OF THE RECONSTRUCITON VIDEOS ARE FOR MASTECTOMY PATIENTS & THEY GLOSS OVER THE WOMEN WHO HAVE A LUMPECTOMY BECAUSE THE TUMOUR IS SMALL AND THEIR BREASTS ARE LARGE SO IT ALL SOUNDS SO GOOD UNTIL. THE BREAST SUREGEON WHO IS REMOVING THE TUMOUR REALIZES THAT FATTY BREASES WON'T HOLS SUTURES WHICH MEANS THEY CANNOT REAPPROXIMATE THE BREAST TISSUE. THAT LEADS TO A LARGE, HARD POCKET OF SCAR TISSUE THAT RADIATION FURTHER COMPRESSES INTO A LUMP THAT IS PAINFUL TO SLEEP ON. THE FIBROTIC BREAST DOESN'T MOVE IT PULLS CONSTANTLY EVEN IF YOU ARE JUST LEANING FORWARD. I HAVE NOT BEEN ABLE TO FIND A VIDEO THAT SHOW OR TALKS EXTENSIVELY ABOUT WHAT CAN BE DONE IN A OBESE PATIENT'S IRRADIATED (SHRUNKEN, HARD & PAINFUL BREAST) TO IMPROVE THE PATIENTS CONDITION. FOR EXAMPLE WOULD DIEP FLAT HELP, WOULD INJECTING FAT INTO THE INTERCOSTAL MUSCLES HELP TO IMPROVE THEIR MOBILITY AND REDUCE THE PAIN WHEN THE PATIENT LAUGHS, OR NEEDS TO TAKE A DEEP BREATH ?????? THIS INFORMATION IS NECESSARY AND GOING TO BECOME EVEN MORE NECESSARY AS OBESITY TAKES OVER NORTH AMERICA. THANK YOU FOR YOUR TIME
Wouldn't the plastic surgeon liposuction out the side fat to make it more flat??
Yes, that’s certainly a technique that plastic surgeons can use for additional contouring.
Wish I would’ve came across this video b4 my DIEP procedure. Doctor made sure to tell me that’s it not a tummy tuck, and didn’t seem open to anything I had to say. So honestly am not thinking about going back to them for my revision… (Any thoughts?)
A second opinion never hurts. I would consider that.
I had stage 2 and had radiation treatment. I waited and focused on getting healthy, best shape of my life. Now I'm excited and ready for this surgery and recovery physically mentally and emotionally.
So very informative...thanks!
Great video thank you SO much!
You’re most welcome Nikki!
Thank you for a very informative video about these options!!!
I am all ears to learn more. When I had my lumpectomy in 2021 it was preceded by as fast research as I could do on "aesthetic flat closure" - is this what the good doctors are speaking of here? Ultimately my most straightforward option was lumpectomy and radiation. My tumor was small (not palpable) and was very lateral in my breast, almost like it was waiting to be plucked out. My breast surgeon is trained in breast conservation but she and I discussed quite a bit that should I have needed a mastectomy, she supported my interest in freeing myself of a breast with cancer in it without creating a substitute breast. I was planning to go flat and not look back (if lumpectomy was not my better option) and it came as a big surprise to me that aesthetic flat closure was not more widely performed. My breast surgeon is partners with another breast surgeon in town who specializes in aesthetic flat closure and she would have joined forces with her partner specialist for the mastectomy I would have had, because she wants more experience in this technique (even though she is a breast conserving specialist). If I have any recurrence at all, I am going aesthetically flat in an eyeblink and now I want to understand what it means to end up as closely as possible to a pre-pubescent smooth chest wall without any excess skin or concavity. Where can I read more? I am a very youthful 66 who feels very feminine and do not need breasts for that, especially if mine keep threatening me with DCIS.
Hi Andrea! We have very comprehensive information about #AestheticFlatClosure in our free app. Here’s a great article to start with: "Mastectomies without Reconstruction (Aesthetic Flat Closure)" prod.breastadvocateapp.com/contents/247
Thanks for this video it brings hope. I have had radiotherapy treatment and the boost left my breast hard with pockets, and in bad shape. My oncologist team doesn't know of a doctor that can perform this surgery for me. I live in Brooklyn, NY and I was happy for find a doctor but unfortunately she's no longer taking new patients. Can you help with any referrals please? Thank you.
Thank you for yet again bringing education to patients, in a very accessible way. I had reconstructive surgery (my surgeon is a superstar in his field, he is also the founder of Breast Advocate 😉) and I now understand I was connected to a T-Stat machine. So, I have a questions for you, if, like it happened for me, one is connected to a T-Stat, why the need for a hourly Doppler test?
I’m 4 weeks after having my diep surgery, I am from the uk so not sure if terms, wording., procedures are different to other countries. I had breast breast cancer a year before my second diagnoses, I had surgery to remove that cancer as well as a breast reduction and lift to both breasts as well as treatment. A year after I had my mammogram when they found out I had triple negative breast cancer again. Iv had my chemo and then my diep surgery. Soon as I was woken up from my diep surgery, I could feel pain in my stomach, but soon disappeared soon as they gave me pain relief and a button on my hand to have morphine every 5 mins though the intravenous line. Though out the night of my diep surgery I had a nurse sit in with my and tested my breasts with those machines they use when listening to type babies heart every 30 mins. This was very tiring as it was though the next day and night. I was too tired to try and get up and walk. So the nurses decided to leave that until tue following day. The following day I got up, I was scared to start with because I didn’t Feel any pain on my breast or tummy, just tightness on my stomach. I managed with the help of a nurse to walk to the bathroom in my room and sit at the sink to have a “cat lick wash” just to fresh up really. The day after I was getting up on my own and was able to have a wee as they top the catheter out of me. (Freedom) I managed to shower on my own, dress and was able to walk the ward on my own. Through out my 6 days in hospital I was able to shower, dress, and go off for walks on my own to the shops down stairs! I didn’t have much knowledge on how I would be after the huge surgery, they put drains either side of my breasts left and right. And two drains on where my tummy had its surgery. They came out the 3rd and last day being the breast ones. Being as I had surgery previously they followed the surgical lines for my new surgery. I requested to have my nipples removed because they never were right after my first surgery and we numb to touch. I called them burt and Ernie as they have different minds to each other and wouldn’t react the same to cold and warm. My stomach is still tight and I have fluttering feelings there kind of like when you first feel your baby move. I find this comforting. I do have “firework boobies” i used this name from the first time I had surgery as you get sharp shooting pains in your boobs. My surgeon told me that the nerve endings are waking up ect so I thought that’s what it would be as I recognise the pain. But check with your health care professional first!!
I hope that my experience helps you, our bodies are different to each others and that is what makes life so amazing. Please remember that you may have other experiences to mine, but hope you can add yours here to give comfort to those in need of it x
Hi hanagh. I am also in the UK and hoping for another reconstruction this time with the tummy method. 30 years ago it was with my back muscle. I have recently been diagnosed again same side. I'm going to ask if they can remove what I have and do another recon. Got my consultation mid September. See what they say.
Hi, I am also in the UK, can I ask you where did you have your procedure? I mean the hospital.
Thank you for this I'm post 6 days for my op I feel like the ken doll at the moment. When I had my mastectomy I had no sensation in my left breast I woke up feeling everything that I didn't before. Boobs are tender but I feel bloated all the time and bruised.
Thank you for sharing this information!!!
Can it be kept in radical modified mastectomy operation where there was no skin saving operation done.
Thank you this was very informative.
I used be a shame of my self 28 years old I feel like a part me was gone . It is true , you do feel better emotionally you got to know your self , after my mastectomy on my right breast I was so scared to take a shower I have a mental break down every day . I was really , really depressed for at least two moths i used to take a shower at night with the light off because I didn’t have the courage to look at my self , 1 moth later I take a shower doing the day I don’t cry that , I eat , I sleep , I laugh , I talk to peoples , I don’t seat in the dark room alone anymore . To anybody that is going through this I send you a lot of hugs & love
