This has been implemented since Metal Gear 1990 on the MSX2 system.

From what I heard this can only happen on Hard mode and it’s mostly caused id stay in the rain or water for too long. This can also happen in MGS1 and MGS3, though it rarely happens.

Taking medicine will prevent snake from sneezing. Not sure if it's available in hard/extreme or European Extreme

This also happens in MGS 1! You can catch a cold from Johnny when you escape torture. I was hiding pretty much like that in the nuclear storage room, and suddenly Snake sneezed right behind the Genome Soldier. My reaction was "OH MY FUCKING GOD. KOJIMA, YOU ARE A GENIIUS!"
