I use to work at Starbucks so I have a little bit of random history facts concerning those size names at Starbucks. So we know the sizes of small,medium and large as "Tall","Grande"and "Venti" at Starbucks. When Starbucks first started out there was a size called "short" which was 6-8 fl oz so the "Short" was the small, the "Tall" was the medium and the "Grande" was the large. when the "Venti" was first introduced it was the extra large. The name came from the Italian word for twenty. After a while Starbucks decided to make the Tall the standard for small, the Grande for medium and Venti the large. While the Short size isn't on the menu, it's still available by request today.

The best approach is to say "I'll have whichever isn't as big as the biggest you do, but not as little as the littlest. If that applies to two or more of the available options not ruled out by my not wanting the biggest or littlest, I'll have the one that isn't as little as the two littlest you do, but is littler than any other." When they've sent a few moments working that out, cheerfully point to someone else's cup and say "That size would be ok". This works especially well if the one you point to turns out to be either the largest or smallest they do.
