It would be relevant and appropriate to include another factor and portray it in some of these educational videos. Thr often obscure hidden contributing factor, such as a spouse nagging or engaging in bickering, ridicule or disparaging comments even prior to arriving at work, sadly, im sure persons had sustained horrific injury or death from accident, who's catalyst was the worker being significantly disturbed or distracted yet not outwardly apparent.

Another very important thing to remember is if someone is lying unconscious in a confined space despite what your instincts may tell you you should not under any circumstances enter the space to preform a rescue unless you are properly trained and are wearing proper equipment such as breathing apparatus. As the person was likely overcome by oxygen deficiency and toxic gases by entering the confined space you will most likely be overcome by the hazardous atmosphere as well thus adding additional victims. It is better to have one victim than two or more as then it will be easier for trained and equipped rescuers to remove and rescue those who are trapped. Remember you can’t help anyone if you yourself become injured or killed. Tragically 60% of people who die in confined spaces are would be rescuers. All too often when someone is overcome by oxygen deficiency and toxic gases untrained and unequipped would be rescuers instinctively enter the space in a attempt to rescue the person often a colleague, friend, or family member but because they weren’t properly trained and weren’t wearing proper equipment such as breathing apparatus they were then themselves overcome by oxygen deficiency and toxic gases. Instead if possible then immediately get a trained and equipped rescue team that is either onsite or nearby and then immediately call 911. That way the rescue team can then rescue those who are trapped without putting anyone else in danger. Then prevent any unqualified, untrained, or unequipped people from entering the space and then leave the operation to properly trained and equipped professionals.
