In this video, we discuss a critical question facing many practitioners today: can ADHD medications still work if the patient is using marijuana? A recent study aimed to answer this by examining how high doses of Adderall XR (80mg daily) affected patients with ADHD and cannabis use disorder. Over a period of three months, researchers compared amphetamine to a placebo to see whether the stimulant would improve both ADHD symptoms and help reduce cannabis use.
The results? Despite the high dose, the amphetamine didn’t improve ADHD symptoms or help patients quit cannabis. This study mirrors previous findings from trials on methylphenidate in teens and atomoxetine, which also showed no improvement in ADHD symptoms or cannabis use.
Though the data isn’t definitive, it suggests that ADHD medications may not work as well when marijuana is involved. The real question is: how much marijuana does it take to block the benefits of a stimulant? We don’t know yet, but the evidence we do have points to a problem. Marijuana has been shown to interfere with executive functioning and can even cause ADHD-like symptoms.
For clinicians, whether to prescribe ADHD meds to a patient using marijuana will depend on individual circumstances. But it's important to inform your patients that their treatment might not be effective while they are still using cannabis. This might come as a surprise, as many patients believe that marijuana helps their ADHD.

    There is one glaringly misleading/inacurate statement in this video: that any individual response to a high dose of an antidepressant is attributable to placebo. You might find in a large population that the average response to a high dose antidepressant is not significantly different than to a low dose. This DOES NOT mean that NO individuals have a differential response to high vs low dose! It means that those folks are in the minority. Thus, for a patient not responding to a low dose antidepressant, without any tolerability issues, it is absolutely worth trying dose escalation, as tolerated.

      I think that this requires a clear informed consent discussion, which most practitioners don't do. Eg "Going higher in the dose of this medication is usually not helpful for most patients and you would have a better chance trying to switch or augment, but a small % of people may be outliers and might respond to high doses. Would you prefer to increase the dose or do something else?"

    Thanks for the video. In the diagnosis of ADHD, one problem is that the criteria used in the some diagnostic manuals unfortunately describe signs as "symptoms". Medically, symptoms are subjective, whereas signs are objective or can be seen from the "outside". Also, many key aspects of ADHD, such as emotional dysregulation, are not even given attention (no pun intended) in the DSM-V, etc. But these are, in fact, true symptoms, and they can't always be observed by an outsider; the doctor has to take the patient at their word e.g. "I am experiencing a deep feeling of rejection because of what somebody just said to me before this appointment. Now my emotions are all over the place". I think the diagnostic manuals today are mostly aimed at diagnosing children, not adults. You aren't going to see many adults jumping on top of tables during a work meeting. Instead, the hyperactivity is usually more of an inward experience. It's sad that the main diagnostic manuals are so aloof as to how ADHD truly manifests in real life.

    Thank you so much for including the cultural history and resulting policy decisions, admitting to bias in prescribing practices tells me how very thoughtful your conclusions and recommendations are, simply a fantastic presentation here for the layperson to understand better, and thereby give better feedback to care providers.     Is this a joke? No advantages in efficacy. You get nothing for combining it with other agents, so useless for augmentation. _And_ it causes QT prolongation? Ain't nothin' cute about that! Why would you ever prescribe or take it? There are older drugs that are safe and effective. If you've literally tried everything, gone through SSRI's, SNRI's, atypical antidepressants, tricyclics, MAOI's... You still have better options than Gepirone.

      You’re not dumb at all! It can be a bit confusing because NMDA antagonists block NMDA receptors, which decreases glutamate activity at those specific receptors. However, in some brain regions, this blockade can lead to a compensatory increase in glutamate release. So technically, NMDA antagonists can both reduce direct transmission at the NMDA receptors and lead to increased glutamate release in other areas. The short answer is: NMDA antagonists decrease glutamate activity at NMDA receptors but can indirectly increase glutamate release elsewhere.

    I enjoy this method of delivery very much. This form of media is easy to digest and I can easily tune in during morning commute. It reminds me of Dnews from nearly a decade ago. If you are guys are open for feedback, I have some suggestions. Better advertise this TH-cam channel on your website. I'm confident there is a demand for this content. Clinicians may just not be aware it exists. List all sources in the video description. I appreciated how screenshots were shared on screen but a link is always classy.

    There is other support in the literature for the anti-depressant/mania thing but it is usually ignored in discussions and reviews

    Methylphenidate sucks and those stealing the amphetamines should be forced to eat Methylphenidate on a daily basis. It causes serotonin syndrome.

    I would love for there to be an affective at home opition for ketamine. I provide esketamine therapy for my patients, but obtaining it for them can be difficult and delays treatment. The specialized pharmacy it ships from often arranges delivery with the patient not me so it is sent back because I am not there to sign for it, and I have had insurance companies only give me a PA for 2 weeks at a time. I find the data on Rexulti interesting, but I have never seen an insurance company that will cover it, unless another AP has been tried first due to the cost.
