Dr. Zane Perkins, Trauma Surgeon for London Air Ambulance and the Royal London Hospital, and Senior Lecturer for the Centre for Trauma Sciences, discusses prehospital thoracotomy in trauma arrest. This rare procedure may be life saving when performed by the right person at the right time under the right circumstances. Dr. Perkins reviews patient selection, injuries, technique, and tool kit for the procedure.

    Cath lab nurse here, I use these push doses everyday, great break down, I personally am not pushing more than 5-10mcgs IV epi but 20mcgs will get you where you need to be haha!

    Hey, can someone explain to me how the concentration of the normal saline became 8mcg/ml after injecting 2mL of the med in the bag?

    Why not add 2.5 ml of 1:1000 to the 250 ml bag instead? Then, you keep the same 10 mcg per ml concentration. Less confusing.

    For the the push dose epi it doesnt matter what fluid is used as long as we have 1 ml of epi. And from there we can push 20mcg from there

    Thoughts on using CPAP or higher peep for intubated patients with 100% FiO2 for the more critical patients to decrease the half-life of CO?

      CPAP may be helpful in creating a tighter seal and therefore increasing the FiO2. A nonrebreather entrains a lot of room air as the patient breathes and provides far from 100% FiO2 as is sometimes taught. The improved alveolar recruitment with CPAP also helps with gas exchange if there is any atelectasis. But CPAP itself does not increase the pressure of oxygen felt at the cellular level.
