Rep of the day 10 - Don't listen to gurus on the internet
Be very careful of believing everything you hear - most people agree with themselves a lot of the time
Rep of the day 9 - Manual therapy facilitates exercise
Manual therapy has a role, but to claim it facilitates exercise doesn't hold up
Rep of the day 8 - Right, maybe we need another scan
We need to find the balance between appropriate and unhelpful investigations for musculoskeletal pain
Rep of the day 7 - Aint nothn that can't be stuffed up by an operation
Navigating the decision for orthopaedic surgery required shared decision making
Rep of the day 6 - Are your ears painted on
We have two ears and one mouth, so we should use them proportionately
Rep of the day 5 - Physios aren't comedians
Being funny or good at engaging with patients is a great start, but hopefully that's not the only string to your bow
Rep of the day 4 - Physios aren't psychologists
Musculoskeletal pain is defined as a sensory and emotional experience, but where do you draw the line with treating people with psychological problems?
Rep of the day 3 - We have hands for a reason
In healthcare, the use of touch can be very powerful for different reasons with different people
Rep of the day 2 - Maybe if we try that just one more time
In healthcare, why do practitioners continue to do the same thing over and over, even when it is clealry not working?
Rep of the day 1 - Death by questionnaire
In healthcare, screening and functional questionnaires are useful, but be wary of doing too many and make sure you put them to use

Wow, what a fancy way of gaslighting this lady. It’s dermatomes reflecting a chemical irritation from inflammation. It’s inflammation pain. She is not ‘sensitized”. She has a sensory system. Nociception is being triggered by something physical. You cannot see it on imaging because it is chemistry,, not broken bones. Keerist, the medical system. Eyeroll

Thanks very much for watching our video and taking the time to comment. The symptoms are not dermatomal - they are not in the distribution of a single dermatome. It could be inflammation, as that is one cause (among many) of increased sensitisation. We are very clear when doing this testing that it does not tell us the cause of the increased sensitivity, rather it helps identify its presence. The interpretation requires consideration of much more information, as sensitisation can be influenced by a myriad of central and/or peripheral factors.

It could be from local spinal issues however, as that can be one cause (among many) of increased sensitisation. at around minute 15 and 17 seconds, Beales refers to a screening tool to indicate the risk of delayed recovery and return to work. What is it? I'm not able to understand what he says, unfortunately.

You pressed harder, so she obviously would have felt it harder. It was even obvious judt watching. I think that's an odd way of testing with the pressure on each side, because you're never going to place the same pressure on each side. The pressure that ends up being given may even relate to your more dominant hand.

    • @painoptions6579
      @painoptions6579 2 ปีที่แล้ว

Thanks Dragonfly on the Wall - interesting you can SEE pressure is different!? Either way, these are clinical tests and the degree of difference between sides should be VERY obvious to the patient in order to justify a difference. We also recommend repeating the test for clarity of response and of course using this as one of a number of indicators of sensitivity, not an isolated test. You can even try pushing much lighter on the 'sensitive' side than the other side and if they person still tells you it hurts / is more sensitive, then maybe there is something in it. Good luck out there. Tim

Thank you very much for the presentation! I am a physio from Mauritius. I visited Curtin University with Manual Concepts in 2016 and I did meet you there for the Masters promotion presentation. Getting such high level evidence based information is really helpful. I am grateful for the the tremendous work and sharing of information. Hopeful to see more videos. Many thanks and kind regards.

Quick logistical question - where do you purchase the von frey filament 2g?

    • @painoptions6579

The originals are expensive. You can search "tactile monofilament" for some other options that can be suitable for clinical use

Hi there Kylie How I understand this situation you have experienced and can validate your experiences from physio, and the awful way that a situation of recovery can be misunderstood and managed. The complexity of trying to manage in a complicated process that is foreign from the usual norm of life with the added disabling pain that can crush your determination to move forward. I feel, and also have experienced and still experience suffering but am glad to hear, excuse my glee i am proud of your achievements. Kudos to you for picking your self up and evolving the new you. Keep smiling as there is hope and Inspiration in your smile.

Awesome video! Based on the mild wind up/increased summation, are you correlating that to a component of central sensitization in this case? I'm all about pain science and always trying to learn more on the subject however I can. Cheers!
