It should never touch the bottom strap. This is how MD Orthopedics shows as proper placement. After the middle strap is placed, the tongue should be pulled away from the buckle to fully straighten out the material so as to avoid a "pinch" that can happen at the buckle area.

I wore the Denis Browne brace until kindergarder in the early 60s. I had a club foot, a forward tortion of the hip and scholiosis. I now suffer constant lower back, hip, foot and knee pain.
