if im not mistaken its when barry drop the rope on that hole opened by the snake fight. he say to you WAIT so he will grab another hope. If you dont wait and proceed to go, you get this scene. Some say this : When you meet up with Barry in the sewer let him go first, but don't follow him, go the other direction. After you meet Enrico and return to the sewer entrance, you'll get this cutscene. Anyway, those are the events that trigger his death somehow. He die in 2 different ways if you dont do "the right thing' to save him
