Informed Consent Checklist
This video contain required elements of informed consent: procedure, indications, benefits, risks, alternatives
• CI-CARE ("C" reminds us to connect with a patient by knocking on the door, calling patients by their proper names, and by truly developing a relationship with each patient as a person. "I" stands for introduce - sharing one's name, role, and purpose for interacting with the patient. The next "C" prompts us to communicate what we are going to do and point out how long we expect it to take. "A" stands for asking permission before examining a patient or taking an action; then "R" prompts a response to questions or concerns." "E" reminds us to have a courteous exit with an explanation of what will happen next.
• Incorporation of 'teach-back' (having the patient describe in his/her own words the procedure, the reason for the procedure, and the risks and benefits)
• Emphasis on simple, nontechnical language
