Sir mera wife ka age 25 hai, uska invasive mucinous carcinoma diagonous huya hai . ER + PR + , Her - , ki67 2% . Surgery huya hai kuch din pehele, nift nodes 12 are negative , Dr. said cancer are not spread any where . Some note from HP Analysis... pT2No .. Tumor size 2.6 cm . Overlying skin are free from tumor . Perineural is not identified. Ductal carcioma in SITU(dcis) solid, papilary, calibriform, intermediate nuclear is presemts within and outside the ivasive component and constituting <25% of tumor. Extensive intraductal component are not seen. Comedo - Necrosis is not identified. All margin of resection are free for carcinoma. Is she need chemotherapy sir , please reply

Myself diagnosed ERPR+ & Her2 + stage 3 , after getting 6 cycles of chemo along with Phesgo, conservative surgery & 16 radiations, getting maintenance dose of Phesgo ( total 12 prescribed out of which 2 infused) I have been given letrozole, CCM, pregabalin 75 & many supplements.still I have pain in my bones & breast pain & stiffness also ,. Hi Dr. Jay !! Amazing educational videos. I watched several. My mom 60 yo has been diagnosed with Early Stage BrCa. Er/pr positive. Her2 equivocal. FISH pending. PET scan and biopsy done. Low grade malignancy in left br. Low uptake in one axilla node. The axilla scares me... And looking for your expertise to avoid ALND. I am trying the best to avoid ALND because of lymphoedema & effects.(Which you have stated as 40% probability ). She's a very active and lively person. And an artist. It is paramount to keep her activities undisturbed as much as possible post surgery for her mental well being. Our most pressing concern is the arm/elbow mobility post surgery. I have done extensive reaearch and watch your videos to improve my understanding. I'd love to learn more about your POV on when exactly is ALND needed, how is that determined (is it only known during the surgery table or before hand) , options to minimize the need for ALND, and if needed then ways to minimise risk. What role does Neo adjuvant therapy play. And maybe elaborate on S-lympha. And it's risk vs rewards. Afaik its a relatively new procedure.
