You have to create the conditions carefully as this would be a global change, and is not recommended to modify OOB Buttons
if you do comment the addExtraField Method it will also work
You're giving the incident number dynamically but it's not best practice. Can I use the gs.getProperty().
Yes, you can use and i have just added incident number for specific condition to show how it works, you can use your property or condition as simple, that's why its sample script man, it was just an example

Good Sharing! Would like to know how would you handle user changing the field "Email to be sent for date" and make sure only the latest changes trigger the notification. E.g. If user do multiple changes to that field, will it create multiple scheduled notifications based on your current configuration?
