It seems like the conversation of cash-pay physical therapy always dilutes to a convo like the Payless conversation. There is always limited insight of the specific differences that can be offered and be valuable with being out of network. It would be nice to hear of the specific value that an out of network clinic is able to provide vs and in network clinic. I do believe that both are necessary, but in network providers always steer the convo to points like it’s all the same and buyers are limited in their knowledge.
You guys talk about RTM at the end. Could you make a video on potentially billing RTM out of network?

I have been offering memberships for 4 years now. Started off super cheap like Tony, all you can eat. At that point I was looking for volume as part of a CrossFit gym. It later evolved to a monthly fee just under $200 that was still all you can eat. I had a handful of people abuse this, mostly because I didn't assert myself as the person in charge of this relationship. Lesson learned. I later revamped it to put a limit on visits, and it provides me with consistent monthly income, while providing tremendous value to my patients. I allow their entire family to share their bank of visits, which provides more value to them, and the monthly fee is lower than the per visit fee. I think the membership model is a great way to provide value to the therapist in recurring monthly revenue, and tremendous value to the patients, by being able to essentially finance a plan of care for themselves and their families with a consistent, reasonable monthly payment.
I felt like this with coaching when I first started. As a professional I learn to never down play someone’s attempt to better or manage their health.

Different demographic Populations. Not every restaurant can be an Italian restaurant. If everybody leaves the sector of geriatric Medicare service the 💩 will hit the 🪭. You are addressing two different different problems. It’s apples to oranges. But thank you for bringing that up. If everybody doesn’t take Medicare then who’s going to see them if they can’t pay. 😉 I’m doing great as a health insurance agent AS a PT. Doesn’t mean PT is going well 🫠🫠🫠🫠

Dave...Do you recall our conversation...Episode??? ... I set not 1 but 2 therapists up as Owner/Directors in 2 of my 12 offices when I was INN...That's another way to retain. talent so that you're not always hiring Triple-A players who move on in 18 to 36 months to the Big Leagues...

Excellent discussion regarding pre employment screening. Self pay clinics should definitely test a provider prior to them working with high value clients. Otherwise, in a normal skill level clinic, I would be concerned with the providers disposition and the overall vibe and culture of the clinic.

Article 28, as it pertains to New York State, does not inherently allow a clinic to cross state lines in its operation. Article 28 is a state-specific regulation under the New York Public Health Law that licenses and regulates healthcare facilities within New York State. The privileges and regulations provided under Article 28 are confined to New York State and are overseen by the New York State Department of Health. For a clinic to operate in multiple states, it would need to comply with the health care regulations and licensing requirements of each respective state. Each state has its own set of laws and regulations governing the operation of healthcare facilities. Therefore, while Article 28 can provide significant benefits within New York State, those benefits and the regulatory framework do not automatically extend to other states. To operate across state lines, a clinic would need to: Obtain Licenses and Certifications: Secure the necessary licenses to operate in each state where it plans to provide services. Comply with State Regulations: Adhere to the health care regulations and standards of each state, which may include different requirements for facility operations, staffing, and patient care. Establish State-Specific Contracts: Develop contracts and agreements that comply with each state’s specific health care reimbursement policies, including Medicaid and private insurance providers. For example, if a clinic licensed under Article 28 in New York wants to open a facility in New Jersey, it would need to comply with New Jersey’s healthcare facility regulations, such as those outlined in the New Jersey Administrative Code (N.J.A.C.) for hospital and outpatient services. Summary: Article 28 benefits are specific to New York State, and a clinic would need to individually meet the regulatory requirements of any other state it wishes to operate in. This includes obtaining appropriate licenses, adhering to state regulations, and establishing necessary contracts for health care services in each state.

I began a poor version of The Patient Experience in 2005...Then in 2016, I'll say I went to The Patient Experience 2.0... In 2018, I believe I perfected what I now call The Patient Experience 3.0 ... The Key Gentlemen is to "Turn The Tables"😉 amongst other things which JD is far more of an expert in doing !
