If you want to improve your grammar, read a lot of books! I got something like 26/40 on my first Toefl and after reading a lot of books in English (about 7 months after) I got 37/40! I didn't even study all the grammar topics again, I just read every time I had the chance. It also improved a lot my score on the reading! (p.s: by the time I took my first Toefl, I had already studied all the grammar topics, but I didn't study everything again to get a better score)
