Hi Diane, its a firefox extention called Screenshot Full Page, basically it just take a long screenshot of a webpage and save it as a png, you can replace it with any design you have to fill in the mock up.

Your totally right Johnson about the logo no longer has its sharp lines around the edges, due to the fact that the logo I used was low res, I did resize it and lose some of the quality. The settings on the Displace horizontal and vertical value will vary on the image and the design/logo that they set on it. Setting the gausian blur can eliminate those and playing around with the blending option without losing too much on the actual look of thedesign/logo

Hi Paula, you can split horizontal or vertical, 03:00 then carefully adjust the points and curve it out those corners or area that are curving, it takes some practice to make it right, just take your time and have some fun doing it, that's a part of your learning curve.
