    I was stressing really bad about two thirds of the way through this lecture. I then called a patient that I discharged with chest pain three days ago, and he's feeling much better. I feel better now.

    1. When in doubt, repeat EKG. 2. Treat women like the elderly.

    Even in medicine women are complicated and uncomprehensible! how to make a diagnose of ACS based on such non-specific symptoms and signs?

    ZEEP is awesome. Allows the patient to fully exhale and also allows for greater comfort since 18/0 is much more comfortable than 22/4 and it's the same pressure support level.

      Matthew L depends on the situation. It would be a terrible idea for someone with hypoxemia with an alveolar filling problem.

    • @matty00926
      @matty00926 4 ปีที่แล้ว

      @@stunnerdoc Definitely agree that it's situational, and that is the sum of what Dr. Mallemat states as well. I just like the fact that there are a vocal few who are willing to state out loud that not everyone needs PEEP. Many docs are afraid to not use PEEP no matter the situation.

    Thank you for your talk, it was wonderful. I work in EMS and we just added ventilators and BiPAP to our box of tricks. We have had CPAP for a long time, but it is nice to be able to offer something more to the patients who need it!

    20:00 It is hard to believe that a massive PE that causes cardiac arrest has serielly negativ Troponins. Hs-Assays detect even intermediate-low risk PE's. Maybe that was because you used old (non high sens.) assays?I have never seen a normal hs Trop T in massive PE (multiple years in a tertiary care centre). Second, see also existing literature PMID: 23205283 and Hakemi et al Chest. 2015 Mar;147(3):685-694. doi: 10.1378/chest.14-0700. Normal hs Troponin savely rules out high risk PE.

    I'm a student so pardon my ignorance, but shouldn't you wait until multiple studies all show the same thing? It needs to be reproducible before you change a whole practice no? You don't want to bounce back and forth changing practices for every latest conflicting study right? Wait until there's a trend of them?
