Tips and Tricks for Semaglutide, Ozempic and Wegovy
Hi, I am Dr. Craig Primack, and today I'm coming to you from Scottsdale Weight Loss Center. It has been quite some time since I've done a medical minute.
Semaglutide
So this weight loss drug is called semaglutide. It is a GLP-1 style drug. What does that mean? So semaglutide comes in two forms. The first form is something called Ozempic, in this blue pen. And the other one is called Wegovy, in the white pen. It is the same drug, although it is a different delivery system.
You and I both have a hormone called GLP-1. It comes from the small intestine, the ileum, the area right past the stomach. The way I think about it is as food goes through the stomach and gets to the small intestine, the body is absorbing it. Even though that's not exactly what it's doing, it kind of says that "I'm full" and sends a hormonal signal to the brain that tells your body "I'm full" so you don't go out and get more food right away, because it's working on absorbing that food right then and there.
The half life, so the amount of time that this hormone lasts in your system, half of it is gone in two minutes. And another quarter of it would be gone in a little bit later. So in a couple minutes, let's call it 10 minutes or less, that hormonal signal is completely gone. This is an artificial form of the drug GLP-1 called semaglutide. And the half life is a week long. It lasts in your system for a week at a time. So your body fuels fullness all the time. That is a great benefit for many of us in weight loss.
Tips and Tricks
So I'm going to show you a few tips and tricks I've learned throughout the last year. The first part is when should I take this drug? It is a once a week drug. When you read the package insert, it takes between one and three days to fully kick in. So I love to have my patients use this drug on a Wednesday. Why on a Wednesday? Because it takes between one and three days, so by Friday night, it's fully kicked in. It's fully kicked in through the rest of your... If there is social eating on the weekend, social eating. And then in the beginning at the lower doses, it may start to wear off before the end of your week. So maybe that's one day before on a Tuesday. Tuesday's typically not a big eating day. So first thing is Wednesday.
Ozempic
So the first one, Ozempic, you actually have to put a needle on. And what I'll show you is it has a cap on it right now. You take the cap off, you take this little needle and there's two pieces of plastic on the needle. You screw it on very simply. There's this first cap you take off right there. There's a second cap that you take off right there. And you guys can't see it, but this is a 32 gauge tiny, tiny needle. The next you do is you look at this little window where there's a zero. The first time you ever use the pen, you prime it by putting it to these first two little dots and you push the button and it goes back down to zero. That's before you even give yourself a dose.
The next time you do it, you turn it all away, let's do it that way, to 0.25, which is about 18 clicks here. These little clicks. That may come into use in the future if we decide to try to adjust your dosing at all. But you put it at 0.25. You put this tiny little needle usually in your abdomen. It can go also in your thigh or arm and you push the button down. And as you can see, it goes all the way back to zero. When it goes back to zero, you then count to 5. 1, 2, 3, 4, 5, and you are done. Find a Sharps container. Or what I tell a lot of people is take an old medicine bottle, the yellow with the safety cap that's empty. Take this little needle off. Just like that. And throw it in your safety container. When it is full, then take it back to your pharmacy. So you do have to use, or at least touch the outside part of the needle on this one.
Wegovy
On Wegovy, another version of this, this is an auto injecting needle. You can see there's a little window here. What I have found is the best place to put this one, even though it can be in your abdomen or your arm, it is in your thigh when you're sitting in a chair. So you pull the plunger out and I'm going to show you the little window and pretend my thigh is my arm right here. And you push it down, you're going to hear two clicks. You're going to hear a click right away. You're going to see this little yellow bar. It's going to go about three quarter. Click again, and then move a little bit more. When it is done moving after the second click you are done, you never saw a needle and you have the full dose in you.
Both of these medicines are adjusted somewhat differently, so please ask your medical professional what is the best way to do that. Hopefully this has helped you a little bit learn something about semaglutide, a great drug for many of us here in weight loss. I will be back soon with another medical minute.

      Constipation is definitely a side effect of Semaglutide containing products. I suggest you speak with your prescribing physician. Those of us who use this drug a lot have many easy tips and tricks for constipation

    What is the number after you’ve reached the top? I’m on number three for the month. But then I’m on Already prepared, semaglutideI at number 4. I stay on one number for a whole month before I step up. And I have no idea how high up these numbers go when you plateau and stay where you are to maintain. Does anyone know? Ps. I am having terrible constipation with it, but no real side effects than that.

      Hi. Not sure what numbers you are referring to. Ozempic and Wegovy are dosed in mg. My suggestion is that you see a physican that specializes in weight loss. If you live in AZ, my practice would be happy to see you. if you are not in AZ, then please visit www.obesitymedicine.org and click on the clinician finder to find someone near you. They would be able to help you out to maximize the weight you are losing or want to lose

      This is a great question. Someone has to be considering all the things that go into their current weight. Consider diet, exercise, sleep, stress, etc. If you have done all that and still desire a little bit of weight loss, the consideration of meds is something that you can discuss with your clinician. Its a much more complex answer than just a yes or no. Good Luck to you.

    The weight loss from topiramate is unmatched (200 mg is ample). Phentermine is just an appetite-suppressant. It doesn’t make you intrinsically lose weight and calories for doing nothing and eating normally like topiramate does.

    number 1 thing that gave me better outcome from wegovy was injecting twice weekly instead of once same dose total weekly but split into 2 times weekly. keeps the apetitte low and 0 cravings :) and yes its safe just make sure you don't go any higher in dose then you already are

    I have been using GLP-1 Semaglutide compound for 6 months and have increased the doses as prescribed. Now I'm taking the 2.4 mg weekly injection but even though my appetite has dropped I'm not losing weight. We didn't eat much even before but with menopause I gained about 40lb on my stomach and hips. Why am I not losing weight? What am I doing wrong? Do I need to follow a specific diet in addition to taking Semaglutide? Please help.

      Hi. I recommend finding an obesity medicine physician or if you are in Arizona give me a call. They will look at your diet, exercise, other medicines , etc and evaluate what you need to do to get you on track.

    I took my Ozempic the first time on December 10, 2023. That was the only time until today. The first time I was deathly sick by the 2nd day and was throwing up, extremely sleepy, a bad headache and it disrupted by sleep. I was sick and unable to eat for at least 5 days. However, I may have, by a bad coincidence, had a touch of a stomach virus. My husband ended up sick a few days later. That being said I was scared to death to take it again until yesterday (Friday) 1-26-24. I was wanting to take a half dose to see what that would do. My pen is marked with the .25 dosage but I counted the clicks to it and there were 36. So I dialed to .25 and then backed it back 15 clicks and injected in my thigh. I feel like I did get the medicine (but I'm still not sure), I had slight headache last night and this morning and my sleep was disrupted like the last time. But no bad vomiting or even stomach upset. My question is: should I take another half dose maybe Tuesday or try to take a full .25 dose next Friday. I'm still nervous to take the full dose. Thanks in advance if someone sees this question :).

      Hi. As I am not your treating doctor, please reach out to your treating doctor and ask them the proper way to dose this. If they are not a resource for you, you need a new doctor. If you are in Arizona, my practice does nothing but stuff like this and would be a good fit. Good Luck!

      Best answer is to find an obesity medicine doctor for a full evaluation and a comprehensive weight loss program. Look at www.obesitymedicine.org and find a clinician part of the website

    I had a reaction to phentermine 37.5 it caused my blood pressure to be very low from day 1 and at the end of the 3rd week of taking my bp dropped to 80/52 dangerously low! Was tired with no energy and winded when standing or walking while on it was super scary! My Dr prescribed Diethylpropion 75 ER and Topiramate at night it’s working great.

      You don’t need a video for that I just went to my primary doctor on December 26 and I have 37.5 mg phentermine and 25 mg of the Topamax

      @@blkqtpy None of the doctors in my state/plan will go along with it. They say "just eat less and exercise more." Never mind the fact that I can only walk about 50 feet. Also, they say "you are overweight because you have no discipline. You need to make better choices. You are weak, you need to act like an adult. I have been trying to lose weight for more than 20 years, and the only real success I had was on PhenFen (which was partly phentermine). Unfortunately, the doctors in my current plan want everything to be holistic: "Give up what you are eating now and just eat alfalfa sprouts and bamboo shoots." None of this is helpful, and I don't have $1600 per month to buy Ozempic or Wegovy out of plan.

      They don't, I tried. They might if you really push it. Because I had severe thyroid issues I ended up going to an encrinologist and he's going to start me up on it.

      You might want to rethink getting prescription meds, because the whole point of the medication is to curb your appetite from wanting to eat anything whenever you want at any time of the day or night

      I’m taking the same combination. I’ve only been on it for three days, but I’m doing just fine on it but you already know that everybody reacts to prescription medication totally different. Someone else is reaction to a med will not be the same reaction that you have and if you do notice that your hair is shedding and coming out then you obviously know that you’re going to have to stop taking the meds by by all means please see your doctor. I saw one lady on here said that she was taking Topamax and after 10 days she’s nervous cause she’s been itching so bad and I’m like why would you continue to takethe prescription without talking to your doctor

    What’s the best way to split doses? Started 3 months ago .25, .5, then 1mg. I also started my dosage on a Wednesday morning because that’s the day I got the prescription and it has worked well, I’m down 23 lbs. For me it starts working about 24 hours after injection. Not hungry in the morning, just a cup of coffee. I eat at around noon then dinner at 6pm. The meals aren’t very big, because it feels like your previous meal is still slow to empty from your stomach. It’s an 18 hour fast each day. What I notice is by Monday, I’m hungry in the morning and Tuesday my appetite has increased even more. Im upping to 2mg this next Wednesday. I feel that 1mg has worked well until the 6th and 7th day, why not split doses to every 4 days. Needles are a one time use.

    I used semaglutide vial and manually injected weekly. Started with 10mg building extra 10mg weekly to 40mg.. I didnt lose my appetite and felt more hungry like every hour. Stopped on 6th week as when injected my tummy would hurt dor a week. Didnt lose weight instead felt very tired. After 2 months, the first 3 areas I injected became infected causing purple lumpy bruising and then big blisters with puss and a hole when drained. Ive put dressing on the area as was very painful. New needless was always used and sanitised. Dr Primack do you know the reason why? I'm on antibiotics now and have stopped the weight loss medication.

    For anyone considering taking Ozempic, do not be afraid of the needle. I put off doing the first injection for weeks after I got mine for the first time. I felt really embarrassed when I finally did it because the needle is so small and painless I had doubts I'd done it correctly.

    Considering the medicine is viable up to 6 weeks could i turn the injector 12 clicks instead of 18 clicks and use the does for 6 weeks ? If i cant find the dose i need and have to buy a bigger dose. Thanks.

      Hi. You may have an idea that others are doing. Please speak to your prescribing physician for ozempic to determine the best way to adjust it. If you doc can’t answer that question, look for a board certified obesity medicine clinician

    Thank you for the video! The first time that I use my Wegovy pen (first week) when I injected I heard a first click and after a second click and my second week When I injected it happened that did not work the same it did so many clicks and all the dashed mark appeared going to 0, I thought it did not worked so I did it again... I think I injected two doses instead of one... it is possible?

      Hi. The pen can be a bit tricky. I recommend my patients to inject it into their anterior thigh. Please ask your prescribing physician what they think is best for you. There is only one dose per pen so you cannot overdose on a single pen.

      Best thing is to discuss this with a obesity medicine clinician. Wegovy also known as semaglutide is approved for weight loss. You do not need to have diabetes.

    Ozempic just about did me in, Had lower back pain, had MRI and was told back is shot, was sent to pain specialist and received shots in spine for 2 months, temp relief for couple days, back did not improve and noticed getting weaker & more tired ( pretty much was more comfortable laying down )after 4 mths I looked up side effects of Ozempic ( for my diabetes ), went from 8.2 to 7.5, seen Ozempic caused my symptoms so stopped taking it, has been 3 wks, lower back and weakness seems to be improving but Pharmacist says it takes 5 wks for it to clear your system. I hope these issues are from the 1.0 mg Ozempic shot and not some type of cancer, will find out when I do blood work the middle of the month at the VA !

    I have been on Ozemptek since March. I am not losing weight at all. I do not understand. Eating 1200 calories a day. I am now on 1.5 dosage

    Wegovy really helped me… I had a really bad relapse with bulimia…I haven’t been bulimic in years 😭 I couldn’t stop eating. Binge eating was taking over my life..and I was always exhausted..it was getting in the way of everything. I was on welbutrin and phentermine and both were ineffective. Wegovy really brought me back and helped me with the weight I gained

    Half the time, using Wegovy, the needle does not penetrate my skin. Sometimes It doesn't even leave a mark. I have to pick up the pooled drug off my skin with a tuberculin syringe, using rigorous sterile technique (I'm a biologist), and inject it manually. It's a delicate operation not to lose the medicine. I wish it just came in simple regular syringes! This is crazy for a drug that is so expensive.

      I am not sure what you are referring to as the “water” but the ozempic pen has a fitting for a standard pen needle. Hope that helps.

    Hey Doctor Primack, many thanks for the video - super helpful. I take 1mg Ozempic but wish to raise my dose in the coming months, so can I technically move across to 2.4mg wegovy or would I have to start again with a low dose of wegovy? Are the interchangeable because in essence they are the same drug, right?

      As always I cannot comment directly on your care but what I can say is that Ozempic and Wegovy are both the same drug= semaglutide but just in different delivery devices. Ozempic comes in several doses as does Wegovy but the doses are not exactly the same. For Ozempic, usually my patients go from 0.25mg to 0.5 to 1mg to 2mg (if needed) and for Wegovy, it also starts at .25, 0.5 then 1mg and then there is a 1.7mg and then a 2.4mg dose. At the end of the day, it depends on several factors as to what you are treating. Ozempic is approved for diabetes and Wegovy is approved for obesity management. Your insurance company may or may not cover each one and a prior auth from your physician may be necessary.

      This is a great question. One of the ways that I address it is by using medical grade meal replacements, not the protein shakes that you get at the grocery store or costco. Otherwise, its very dependent on you, your medicines and what your diet is lacking. Much more complex than a post on a youTube video. 🙂

Because one of the benefits of medical-grade meal replacements are that you can use them as a full meal replacement (replace all your food for a period of time) usually at a very reduced calorie level (that should be monitored by a physician for safety), you cannot buy these meal replacements from the grocery store. Grocery store meal replacements are not made to replace more than a meal or 2 each day. They do not usually have all the required nutrients to support your health. Medical grade are just that, provided by a trained medical professional, optimally who is certified by the American Board of Obesity Medicine and who will provide you a comprehensive weight loss plan.

    Never again! I took the first shot on a Thursday afternoon. By Saturday morning I began vomiting and continued to vomit for 18 hrs straight. My husband was out of town and I was alone with my young daughter. I considered calling an ambulance. Literally, I vomited for 18 hrs straight!!! I felt terrible for over a week. Warning to the wary. This drug is not worth the risk.

      Well from my understanding ozempic is for diabetes patients. Wegovy is for weight loss. Same drug but different forms/brands. Semaglutide isn't only for diabetes.

      0.25mg is the starter dose. some people (who are very sensitive to this med), feel it right away. Some take a while and there are a few people who dont respond. Please speak with your physician about what you are feeling (or not feeling) and come up with a good treatment plan.

      Please ask your physician to help you with diet. 1) your calories may not be low enough for your metabolism 2) you may not actually be on 1500 calories 3) something else specific to you and your health/medicines/sleep.     I started Ozempic about18 months ago and over 6 months I lost 14 kgs. Then unscrupulous doctors began to prescribe it for their patients and so there was none available in 🇦🇺. I have started again now April 2023 after having gained 3 kgs.

      The supply problem impacted many other countries too... not sure about "unscrupulous" doctors when the problem really started (even WITHOUT prescriptions for diabetes or obesity) with people paying full out of pocket, and they were "first serve" before insurance patients or REAL prescribed patients... Sorry to hear that u regain some of the lost weight. In my case, I have lost all the extra weight I had lost (almost 27kgs) in 9 months, and my A1C and glucose levels are perfect. My appointment is next week and I am not sure in which direction my doctor will lead me to.

      hi. I cannot comment specifically nor have any obvious reasons that your doctor would have recommended that. Please ask them specifically as they know your medical background. For a general recommendation, I almost NEVER recommend that my patients DON'T exercise during weight loss or maintenance. Usually but not always, more exercise is better.

      As a general recommendation, there is no contraindication to the combo of Ozempic with phentermine. I can’t recommend it for you as I don’t know your medical history, but as always, talking to your obesity medicine specialist is the best way to proceed.

    Dr. Primack.. thank you for your video. I am on my 10th week of semaglutide. I have only lost 9 lbs. My dr explains that I gained muscle. I was 163 now I am 153. I need to lose at least 20 more lbs. I am on a .75 dose as she has increased it each month. The worst side effect is heartburn...all the time. I don't have much of an appetite which is great. My meal sizes are drastically reduced. How long do you think it will be before I see significant weightloss. I would love your opinion. Thank you, Linda

      Linda, best answer is that I don’t know. The medicine decreases appetite and the diet is why You lose weight. I get pretty aggressive with diets and my patients lose rather quick. Please ask your physician to help you predict. All people at Scottsdale weight loss lose an average of 12 lbs the first month as an example. Good luck

      Aloe Vera juice from your health food store helped me with heartburn. Take Semaglutide on Wednesday night so it is mitigated while you sleep. My tips, these helped me.

I usually will recommend one of those as we know the benefits and side effects. Please discuss this with your prescribing physician. If they use a lot of semaglutide in their patients, they should have a LOT of experience with this.

    So I am taking Wagovy now for 2 weeks.. I take it Wednesday night however I get moderate gas or burping some water retention a little bit of stomach pain but the acid reflux is terrible.. what would be the Fix for that? It keeps me up all night and very painful

I take .25 for 2weeks now & I take it on Wednesday a few hours after that shot I get super bloated, it can get painful, sometimes I roll onto my ecesrsize ball to push down on my stomach, I'll also try alovera as sometimes i get heartburn & I'll go back to baking soda & warm water to help get out the air ... I've been trying real hard to drink a gallon of water a day sometime I do sometimes I don't. I'm considering going up to .5 this coming week. Hope it'll keep working or work faster.

    For my latest dose, my pen did not leave any sort of puncture mark on the skin of my abdomen, just a circle from the outer ring. The plunger also moved much more slowly than usual. Is it safe to assume that it did not inject properly? In the past, there have been a couple times when I did not feel the effects of the Wegovy at all for that week, so I believe it is not consistently injecting.

      As a general rule, I think the Wegovy pen is best injected sitting in a chair and injecting into one's thigh. The stomach is too soft to have the pen properly inject all the time.

    One huge difference is you have to push Wegovy pen extremely hard into your leg or arm. It will not work in my stomach.

      I did not have any trouble with the syringe. You can't be cavalier or non-committal with the syringe. You have seen many injections given you by various medical professionals. Be prudent and forceful with the injection as if you are giving yourself an important shot. Once the shot is finished check the site for leakage or a slight drop of blood and you will be fine.
