The Aegis VII Incident
Desperate and on the verge of extinction, humanity cast out into the void of space to discover rich, new worlds waiting to be harvested. All of Earth became devoted to this effort, and the Planet Crackers were built to further this goal. Eventually greedy eyes fell upon Aegis VII, restricted for centuries for reasons unknown. There, buried beneath its surface, lay an artifact, the Red Marker. The USG Ishimura, first and most famous of the Planet Cracker class vessels, was dispatched to retrieve it. The Red Marker, thought to be the key to human immortality, brought not salvation...but doom.

By the way, losing an eye like Asuka still is the most seriously visible injury Kiana had suffered in 7 years (not counting the Zombification in chapter 16 of the main story).
