Weight loss strategy in over-weight and obesity has been uncertain for decades in regards of CV outcome, until the emergence of convincing benefit of bariatric surgery (sleeve gastrectomy, Roux-en-Y gastric bypass) in observational studies. Recognized as a “disease”, not a “lifestyle choice”, obesity and obesity-related CV comorbidity (mainly HFpEF) are now successfully managed by intensive lifestyle intervention (exercise, dietary), novel GLP-1 receptor agonist especially semaglutide as well as novel GLP-1 & GIP dual receptor agonist (tirzapatide) in addition to bariatric surgery, all with evidences of CV outcome improvement. In the era of obesity pandemic, we as cardiologists should be ready and well-equipped to fight against over-weight, obesity and obesity-related cardiovascular complications, not only HFpEF.
